Summary


A new exciting opportunity has arisen to join a well established community epilepsy nursing team. You will be well supported by the Epilepsy Nurse Specialist and the Assistant Practitioner within the team to deliver community care for patients with epilepsy.

To assess, evaluate and deliver programmes of nursing care for the epilepsy specialist caseload of children with nursing needs in the community of Luton and South Bedfordshire.

The purpose of this role is to facilitate early discharge and to prevent admission/readmission to hospital and to develop selfcare for caseload of children by providing Support nearer to home.

To support the lead nurse in service provision in support of children, young people and families who have increased needs with regards to their epilepsy management.

You will be supported by the Epilepsy Nurse Specialist to assess, plan, implement and evaluate individual programmes of care for a caseload of patients requiring treatment for Epilepsy.

To support the transition of young people from paediatric to adult care, this may include attendance at transition clinics, liaising with the adult neurology team and in-reach to adult wards as appropriate, for young people in the process of transition.

Liaise with multidisciplinary teams within hospital (paediatric and adult), community, primary, and social care, including schools and nurseries. Patient contact- work in partnership with children, families and carers to provide the optimum individualised nursing care in the most appropriate settings eg. Home, school, community clinic.

Policy development- to participate proactively with the Epilepsy Nurse Specialist to develop and improve the service. To update guidelines and Standard Operating Procedures as required. Maintain reporting of relevant data to national surveillance projects and local audits.

Educational

To provide specialist guidance and ongoing support to the healthcare providers involved in the care of patients diagnosed with epilepsy.

Identify own education and development needs, access appropriate learning opportunities to ensure clinical knowledge is continuously updated.

Participate in clinical supervision / preceptorship / mentorship as required.

The post holder will support the development and delivery of training /education programmes to healthcare /other professionals, including visits to schools, nurseries and playgroups, where appropriate to ensure that other professions eg teachers have a basic understanding of epilepsy to ensure confidence.

To assess and evaluate the health education needs of the children and their carers about their condition including medication and treatment and to provide verbal and written information about the conditions which is age appropriate and culturally appropriate.

To support the lead nurse in the training of doctors, nurses, teachers and other healthcare and external agencies in both formal and informal settings. This includes supporting the lead nurse in the organisation and participation in study days and conferences.

To participate and present in regional, national, and international workshops and conferences.

Establish links with other specialist nurses and voluntary organisations to promote development of the epilepsy service.

Communication

To effectively communicate with children, young people and their carers, providing and receiving highly complex, sensitive or contentious information regarding health history and current health status, using persuasive, motivational, negotiating, empathetic and reassurance skills where required to undertake an assessment of children and young people with Epilepsy.

Communicate sensitive information concerning the patient’s condition, which requires persuasive and reassuring skills; some patients may have special needs, learning disabilities and therefore communication with the child and parents is key.

Work with Interpreting services as appropriate where there are difficulties with language and communication.

Work with other services and professionals such as acute consultant paediatricians, acute hospital staff, children physiotherapists, GPs and practice nurses, education staff, other community health professionals. Liaise with consultant paediatrician epilepsy lead to provide continuity of care for shared patients. Work closely together to achieve better outcomes for children with epilepsy.

Ensure records of patient consultations are accurate and meet the standard for record keeping required.

Budgetary Responsibility

Aware of service budget and supports Team Leads to stay within budget.

Human Resources

Participate when required in the recruitment and appraisal of other members of the team.

Maintaining own professional development and requirement to take part in appraisal and KSF process.

Research & Development Activity

To participate in clinical audit to ensure continuous development and improvement of a quality service in line with Clinical Governance.

To assist in the monitoring the quality of the service and contribute to regional and national forums in the development of audit and quality systems.

To work within evidence-based practice and national guidance (NICE, Clinical Assessment Tools).

Complete staff surveys.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes.

The post holder must adhere to infection control policies and procedures.

It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

Provide patients and their families /carers with information on standards they should expect from the team.

Participate in clinical supervision on a regular basis.

The post holder is required to participate in relevant emergency preparedness process for their team.

The post holder must assist in monitoring the quality of the service and contribute to regional and national forums in the development of audit and quality systems.
