Summary

Location: Heather Grove, Hartlepool, TS24 8QZ. This post will require working across sites in Hartlepool and East Durham based on service delivery needs.

Hours: 30 hours per week (Monday – Saturday) on a rota basis

To be part of a caring team, providing a wide range of activities and support to people with a dementia, including transporting and assisting them to and from the day centres.

Salary: £11.44 per hour

An exciting opportunity has arisen to join the community-based services team at The Hospital of God. Your role as a Driver Care Assistant will be to work across Hartlepool Day Centre and Minerva House in Horden (and other community based services as required), to provide a wide range of meaningful activities and support to people with a dementia. You will work across the two centres, depending on the needs of the services, covering driver and care assistants’ absence.

The work can be physically and mentally demanding, and the person appointed to the post will be expected to have a good understanding of caring for people with a diagnosis of a dementia or related illness or memory impairment.

Care for people must be provided in a way which preserves dignity, treats individuals with respect, offers choice and safeguards privacy.

Our services play a critical part in delivering support to reduce social isolation, improve and maintain a person’s health and wellbeing and empower people to have more independence, autonomy, choice and control in their lives and shall be delivered in a flexible manner tailored to meet the outcomes of people.

The Driver Care Assistant role is part of a wider team which will:

Encourage, assist and support people to be socially included, therefore reducing isolation, build confidence and develop new friendships.
Develop, deliver, monitor and review bespoke outcome-focused support programmes as determined and prescribed by people.
Provide interesting, varied and stimulating activities in response to the choices and interests of people.
Provide sessions for people in a group environment or on an individual one-to-one basis
Consult with people and their representatives to ensure they are involved in service design and are able to influence service delivery

Responsibilities:

Whilst covering the driver responsibilities you will be responsible for transport and safety of guests from home to the day centre and home at the end of each day, or to another community based venue as required.
Providing care duties associated with transport services. For example, assisting passengers to prepare for the journey and to and from and on and off the vehicle and ensuring passengers are safe and comfortable when on the vehicle
General maintenance and cleaning of the bus, reporting all faults to the relevant Manager.
To complete daily mileage and service records for the bus, which are to be checked by the relevant Manager
To organise with the garage to ensure the maintenance of the bus is carried out.
Plan bus list rota in conjunction with the Manager.
Support and train care staff, trainees and volunteers in the use of the tailgate lift and positioning of wheelchairs and clamps on the bus.
Carryout the relevant risk assessment on the use of the tailgate lifts.
Restrict the use of the tailgate life to anyone who is not trained to use it.
Undertaking other driving and attendant duties as and when require
To work as allocated by the Service Manager to cover all day centres in the absence of staff members

Care duties

To help people to lead as full a life as possible and to organise activities e.g. crafts, games, reading, reminisce etc.
To be part of a network of services within Hospital of God in the community.
To provide personal and social care to people in accordance with needs.
To ensure care plan and risk assessments are updated and referred to at all times.
To administer medication if requested provided an agreement and consent to administer prescribed medication has been signed by person.
To encourage people to retain their independence by signposting them to appropriate support services.
To help with serving of meals, refreshments and assisting people where necessary. Helping to clear away the dishes afterwards.
To assist other members of staff if necessary.
To report any changes in people’s health & wellbeing to the Manager
