Job title: Principal Pharmacist – Cardiology and Heart Failure - 9 month Maternity Leave Cover.

To lead, strategically develop and deliver a consistently high-quality clinical pharmacy service to the Cardiology service across all sites of the Royal Free London NHS Trust. To lead on the provision of pharmacy services to the Heart Failure Team, including participation in multidisciplinary ward rounds and team meetings. Working closely with the multidisciplinary team and divisional managers the postholder will develop initiatives and guidelines to ensure the safe and cost-effective prescribing of medicines within the specialist area to optimise clinical outcomes and minimise adverse reactions and errors. They will work with a high degree of autonomy, reviewing own caseload to optimise patient care within the specialist area. Identify and lead on quality improvement projects to enhance the safety, efficacy and/or efficiency of services.

The principal pharmacist for cardiology and heart failure will lead the provision and development of the clinical pharmacy service in both inpatient and outpatient settings (via the Pharmacist-led Heart Failure Clinic).

The successful candidate will provide regular input in consultant led ward rounds, participate in the decision-making process and provide expert specialist advice on the effective use of medicines amongst this patient group.

The post holder will be required to manage medications used in the Heart Failure service with provision of expert prescribing advice in the specialist area.

The Pharmacy department is a friendly, progressive and expanding unit dedicated to providing the highest standards of pharmaceutical care and service. Providing a variety of pharmacy services across all three sites, the pharmacy department have recently played a pivotal role in the implementation of electronic prescribing system and have strategic plans for the development of more consultant and independent prescribing pharmacists. The pharmacy cardiology team play a key part on providing cost-effective and timely service to patients either throughout their stay as inpatients or through pharmacist-led clinics. The team are well-respected both at ward and clinic level, providing advice to both nursing and medical staff on the use of medicines for this complex set of patients.

To lead the provision and development of high quality clinical pharmacy service as part of the multidisciplinary team to Cardiology and Heart Failure patients(s), to ensure maximum safety and economic drug use and to demonstrate professional accountability to enhance patient care. The responsibilities will include:
• Regular participation in consultant ward rounds with significant input into the decision making processes, taking legal and professional responsibility for the decisions
• Provision of expert specialist knowledge in the field(s) of Cardiology and Heart Failure
• Demonstration of advanced level of reasoning and judgement
• To accept referrals from other pharmacists, consultants and the multi-disciplinary team to review Heart Failure patients and optimise medication use. To challenge treatment choices. To refer to consultants where required
• Delivery of a high quality, specialist out-patient and ward-based pharmacy service. This involves medicines reconciliation, drug supply, review of highly complex medication regimens, interaction monitoring, clarification of medication changes made during stay, drug interaction and adverse drug reaction monitoring.
• Managing medications used in the Heart Failure service . Provision of expert prescribing advice in the specialist area including selection, adverse events and dosage adjustment in specialist area where limited or conflicting information is available. This advice may be challenged.
1. Communication of highly complex pharmaceutical information, according to the level of understanding, direct to patients/carers and healthcare professionals.
2. Liaising and working with other specialist pharmacists to share best practice, and strengthen patient focused pharmaceutical care• Proactively identifying medicines management issues and preparing pharmaceutical care plans to address the issues identified.
• Leading the development and ensuring adherence to evidence based prescribing guidelines/protocols within the specialist areas
• Development and review of patient group directions and shared care protocols
• Promotion of high quality cost-effective prescribing
• Making evidence based decisions, in partnership with patients and other professionals within the specialist area
• Making clinical decisions where limited or conflicting information is available
• In conjunction with the clinicians and managers, ensure compliance with CQC medicines related outcomes, NICE technology appraisals and other government initiatives and national policy
3. Implementation of relevant local and national guidelines within the specialist area
4. Implementation and evaluation of Drug Usage Reviews and clinical audits, and changes in prescribing practice
5. Being actively involved in the development of relevant patient information leaflets which complement licensed product information• To be an independent prescriber and use this skill to enhance medicine prescribing within the fields of Cardiology and Heart Failure
• To be an active member of the Heart Failure Team (HFT), duties include:1. Demonstrating expert clinical knowledge in the field of Heart Failure
2. Accepting new referrals for heart failure patients and assessing appropriateness of medication in conjunction with consultant
3. Prescribing in conjunction with the HFT and referring clinical team
4. Ongoing monitoring of patients including blood / electrolyte monitoring
5. Development of guidelines
6. Review of available medications including selection of most cost-effective options
7. Ongoing clinical audit of HFT activities
8. Member of the North London Cardiology Pharmacist Group


7. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT
• To be responsible for identifying, prioritising and implementing long term strategic plans for developing the clinical pharmacy service to Cardiology and Heart Failure
• To identify and promote best practice
• To lead on the continuing development and implementation of policies and guidelines to ensure that medicine prescribing within the specialist area complies with existing legal requirements and best practice in the field.
• Responsible for proposing and implementing changes in practice, policy or procedure relating to medicines management in the specialist areas and the local health economy
1. Ensuring compliance with CQC medicine related outcomes, NICE appraisals and other government related initiatives• To identify and assess opportunities for expanding pharmacist involvement in clinical activities
• To develop and collect data on KPI’s relating to medicine prescribing within the specialist area and prepare regular reports for dissemination to relevant committees and specialities.
• To regularly audit the use of medicines with the specialist area including ensuring compliance with NICE guidelines. To feedback the results of these to other members of the multidisciplinary team and implement any improvements into clinical practice
2. To assist in the implementation of the Trusts Clinical Governance agenda within Cardiology and Heart Failure• To be an active member of the senior pharmacy team and participate in the development and implementation of the Trust pharmacy strategy and Hospital Pharmacy Transformation Programme


10. RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES
• To demonstrate effective control of expenditure on medicines within the specialist area, monitoring costs and devising strategies to ensure the most cost-effective use of resources
1. Provision of detailed evaluated reports on Cardiology & Heart Failure drug expenditure to the Divisional Finance Manager, General Manager, and Clinical Head of Service• Forecasting new drug developments and identifying changes in prescribing practice that may impact on the drugs budget
• To work with the relevant stakeholders in developing a model of care for the introduction and management of new medicines within the specialist area (e.g. Heart Failure) working closely with NHS England where necessary
• Identify potential cost savings associated with changing medication use within the specialist area
• Working closely with the commissioning department, to ensure funding is in place for high cost drugs within the specialist area.
2. Liaising with the key decision makers (senior managers, CRG’s, commissioners) to ensure provision of funding for high cost drugs within Cardiology and Heart Failure and maintenance of the high cost drugs database (PHRED)• Interpreting published data on new drugs and to prepare evaluations and present applications to the Drugs and Therapeutics or Joint Formulary Committees
• To formulate business plans to support service development within Cardiology and Heart Failure
• To oversee the delivery of homecare services within the specialist areas
• To contribute to the delivery of the Trusts saving plans whilst maintaining quality and safety in all aspects of medicines management


13. RESPONSIBILITY FOR RESEARCH AND DEVELOPMENT
• To undertake and co-ordinate clinical research and audit project and publish or present these findings. To work collaboratively with other pharmacists to co-ordinate research
• To support the clinical trials pharmacist in the management of clinical trials relating to Cardiology and Heart Failure medicine use
• To supervise and advise other members of the pharmacy team research projects
• To support clinical research and to monitor the relevant clinical trials protocols submitted to the ethical committee within the specialist area
• Develop an annual research and audit programme related to medicine use within Cardiology and Heart Failure. To actively present drug related/pharmacy practice based research at national and international conferences.
• To seek funding to support research through sponsorship and research grant applications
1. EDUCATIONAL RESPONSIBILITIES
• To act as a resource for Cardiology and Heart Failureprescribing issues across the trust and community where appropriate
• To motivate and inspire staff and gain the trust and cooperation of other members of the multidisciplinary team
• To provide advice on Cardiology and Heart Failuremedicine prescribing and use to both pharmacy staff and the wider healthcare team.
• To develop training resources for pharmacy staff and the wider healthcare team on Cardiology/Heart Failure medicines and the prescribing of these
• To contribute to the departmental appraisal and development programme for junior pharmacists. This includes:
• Provision and evaluation of the induction and competency based training of junior pharmacy staff rotating through specialist area
• Participating in the departmental teaching commitments
• Acting as a tutor for diploma / MSc students
• To facilitate pre-registration trainee pharmacist projects


• To contribute to and participate in the education and training of pharmacy, medical, nursing staff and other healthcare workers
• Deliver teaching sessions to medical students at the Royal Free Medical School
• To work with the Royal Pharmaceutical Society, General Pharmaceutical Council and HEI’s to define training and requirements for future generations of Cardiology/Heart Failure pharmacists



ADDITIONAL DUTIES / RESPONSIBILITIES

To participate in the weekday, late duty, oncall, weekend and Bank holiday service rosters as required.
• Other relevant duties as assigned by the Chief Pharmacist, or any other manager or head of department.
• The postholder will be expected to work across the trust sites
