Summary


An exciting Case Manager opportunity has arisen for a registered nurse to join our small and friendly team on secondment based at Wymondham Health Centre Norfolk.

The Frail and Elderly Team provides Case Management support to patients at home with long term conditions who are at high risk of acute admission due to health and complex personal needs.

You will assess and engage patients in self-management of their conditions, linking with key stakeholders to achieve a positive outcome for the patient. The ability to encourage and motivate others is a key essential for this role.

You will attend Monthly MDT meetings at the local GP surgeries

Previous experience in a similar role would be an advantage, with particular reference to the management of long term conditions.

Applicants will be required to work using mobile technology and will need to be flexible and able to work across the South Locality as required therefore applicants must be able to provide their own transport

Wymondham is an attractive historic Norfolk market town, 9.5 miles South west of Norwich with good bus and train access.

Assess, plan, implement and evaluate all aspects of patient care, and develop treatment/care plans that may need to be delivered from a range of options.
Following a holistic assessment, devise an individualised care programme for each patient, modifying it as required.
Be aware of Assistive Technology and utilise where required.
Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
Be required to use tact and persuasive skills in order to gain the patient’s co-operation in their treatment/care/management plan.
Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
Contribute to Personal Health Plans as required.
Be required to undertake risk assessments and act upon them appropriately.

Case Manager

To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold
Standard Framework.
Plan and organise specialist services within the integrated team.
Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis.
Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
Order equipment and supplies via the electronic ordering systems used within the team.
Ensure informed consent is obtained prior to initiating interventions.
Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
