An exciting opportunity has arisen to join a new innovative team within the Serious Mental Illness (SMI) and frailty pathway in NE Essex. We are looking to recruit a motivated, hardworking team leader who has excellent communication skills with a passion in driving service improvement and working collaboratively with other health care providers and third sector agencies. The successful candidate will be responsible for the delivery of a high level service in providing a proactive assessment, monitoring and management of patients over the age of 70 years with an SMI and the management of the team. As part of the national agenda for transformation the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting by integrated working with the community teams within the local neighbourhood model in line with the North East Essex Alliance.

Develop knowledge of frailty and serious mental illness in order to function as an informed lead for this pathway

Responsible for the overall delivery of a range of evidence based interventions to service users based on identified needs, review the effectiveness of care packages and discharge service users back to primary care / other providers in a timely manner.

This is a developmental role where the team lead and team will be required to be adaptable to changes within service development.

Develop robust interagency relationships with key stakeholders supporting development of the integrated care pathways and locality working.

Responsible for the coordination and management of the team in providing a safe, effective and therapeutic clinical service, ensuring the delivery of high quality community mental health care packages.

Responsible for general service management including
• Appraisals
• Supervisions
• Complaints/investigation management
• Management of sickness absence/disciplinary/capability and other workforce procedures and policies.

Support others in their learning and development in relation to the management needs of patients with serious mental illness and frailty

To ensure the service works in line with CPA/Social carer/risk management/ safeguarding and other relevant policies.

To lead on the interface between primary and secondary care services and across the North East Essex Alliance care pathways and groups in order to reduce barriers and access difficulties.

The focus of this role is to lead, support and facilitate the development of integrated Neighbourhood SMI and SMI with Frailty pathways. Working in partnership with the wider NEE Alliance health and social care economy including third sector organisations. Working to minimize inappropriate admissions to both general hospital and mental health inpatient settings. You will be expected to work collaboratively with service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services

You will have continuing responsibility for proactive assessment, monitoring and management of patients and ensuring care is current, evidence based and in line with practice standards. You will provide clinical leadership to the team in evaluation and monitoring of the service, provide specialist assessment as per the service users, and carer’s requirement
