Summary
Key Responsibilities: Clinical To be a dietetic advanced specialist for diabetes for the diabetes service, working as an autonomous clinical practitioner through direct referral, comprehensive client centered assessments, specific interventions and evaluation of practice. Demonstrate a high level of advanced clinical competence in diabetes management; based on in depth knowledge, research and evidence-based findings Development and delivery of training, and supervision for other health care professionals including new staff to the team, primary care teams and students. Be responsible and accountable for a caseload of new and complex patients for a limited time, liaising with all members of the multi-professional team and colleagues in primary care. Deliver structured education and supported self-management programmes as appropriate Provide evidence based clinical advice on diabetes management to people with diabetes, their carers and health care professionals. To work to promote good practice in diabetes care locally by offering leadership, guidance, education, and mentorship to primary health care professionals and other dietitians. Plan, review and support patients individually within an outpatient setting or domiciliary visit and also deliver structured patient education for people with complex needs and uncontrolled diabetes. Support will also be directed to patients who require further self-management education. Support people with diabetes and their carers to achieve individualized goals and care planning to maintain their physical and psychological well-being, encouraging self-management. Support the clinical lead to develop policies for diabetes care for Ealing. Participate in clinical supervision within the Diabetes MDT and within Nutrition and Dietetics for personal learning and development as well as supporting the dietitians providing different services across Ealing Community Partners Network with other Diabetes dietitians in CNWL and beyond Management/ Service Development To contribute to implementing the dietetic aspects of relevant national and local guidance documents for diabetes services within Ealing Ensuring adequate and safe provision of services that meet client need statutory requirements and professional responsibilities. Working with wider diabetes team to support and develop community diabetes services Support the long-term strategy for diabetes care, ensuring involvement from key stakeholders, commissioners and experts in diabetes and self-management support to steer, support and validate the work in this area. To initiate change effectively in response to clinical and organizational demands Report and account for activity through Key Performance Indicators and clinical outcomes. Educational Support and report operationally to the Diabetes Matron to identify educational requirements for patients, carers, and professionals. Support the DICE in the delivery of diabetes courses at different levels for trained and untrained staff with particular emphasis on improving the quality of diabetes care. Lead and participate in group education programmes. This will be for people with diabetes at different times in their disease process i.e. newly diagnosed Type 2, insulin initiation and ongoing care, to ensure that both physical and psychological aspects of diabetes self-management are addressed. Contribute when needed to ongoing peer support initiatives such local Diabetes UK. Maintain and update own knowledge and expertise by attending conferences, workshops, and study days. Always act in accordance with HPC Codes of Professional Conduct and Scope of Professional Practice. Participate in Dietetic Student Training across the wider CNWL dietetic teams.
