Summary


Southwark is currently implementing service transformation within its community mental health services. The transformation is about enhancing the quality of our service delivery, improving the experiences of those using our services, maintaining staff morale and to ensure services are meeting the NHS Long Term Plan. It is an exciting time to be part of this transformation in Southwark and you will be directly involved in improving ways of working with investment in resources to enhance services. The new teams are no longer based on diagnosis and will work with all individuals from a particular locality who have mental health needs that require support from secondary care. We are looking for two Band 7 Advanced Practitioners to support with the transferring of patients between community teams, working with the Primary Care Networks and overseeing flow within the team. The Advanced Practitioner will join a thriving team of supportive, committed and skilled practitioners from a diverse professional background who are dedicated to changing the future outcomes for adults who present to secondary mental health services. They will work closely with the Team Manager, Consultant and clinical service lead to ensure clinically safety and support for service users.

The role of the Advanced Practitioner will ensure that the care delivered by the team will be evidence based and provided in a compassionate and caring way. The role will support the Community Matron and Head of Nursing to drive the programme for Quality and Professional standards within the team and ensuring that the delivery of high quality care consistent with the CQC fundamental standards, Safeguarding principles, Trust values and the Trust and operational strategic objectives.

The post holder will have advanced clinical expertise in assessing, working and caring for Adults with complex mental health needs in a community setting.

Our team bases in Southwark are Chaucer Resource Centre , Surrey Quays, St Giles, Camberwell and Marina House in Camberwell.

You will be supported with regular supervision, appraisal and a personal development programme.

Clinical

To act as a clinical lead in the team, both informally when providing guidance to colleagues, and in Multi-Disciplinary Team meetings when crucial clinical decisions are being made.

To participate and contribute to team senior management meetings with Consultant, Lead Psychologist and Team Leader.

To support patients to transition from secondary care to primary care by providing appropriate number of step down sessions at the point of step down from secondary care services dependant on patient need.

To work closely and in partnership with colleagues within primary care and their community partner organisations a collaborative way, ensuring that decisions are made that ensure the best care for service users and that there is a seamless pathway.

To support GPs in assessing risk by providing expert advice and consultation.

Care Coordinating Responsibilities –

Assessment: To formulate integrated needs assessments for service users requiring commissioned services and to monitor care packages as directed. To undertake risk assessments regularly, in liaison with all other health professionals and agencies involved. To ensure that risk assessments are regularly updated, are meaningful and are reflected in care plans.

Engagement: To work with service users who are difficult to engage in the discharge process. To support the care coordinator to engage & encourage service user participation in treatment delivery as well as facilitating independence.

Support:To support with on-going care co-ordination during transfer process within the CPA framework of a designated caseload of service users who are experiencing severe and enduring mental illness. To ensure that the Trust and Social Services CPA and Care Management protocols are followed and standards met. To provide support and advice in relation to service users regarding the issues concerning them. To work collaboratively with service users focussing on a recovery approach. Provide information to help service users.

To be aware of and adhere to all Trust Policies and act as a role model to other staff and meet the required professional standards for ongoing registration.

To deputise for the team leader when required.
