Summary
To follow process set out by the DoH on the National Framework for CHC & FNC and to provide comprehensive clinical advice, assessment, case management, review and follow up on all CHC & FNC functions for the East Riding Make presentations to the Joint Commissioning Panel of complex cases where individuals may meet Continuing Health criteria for funding. This may also include presenting clients who would not meet Continuing Healthcare criteria for funding. Provide effective liaison with all local community trusts, and local authorities including those in other areas as necessary to facilitate the interface between acute and community services and build on existing interagency networks. Collate, analyse and summarise clinical health and social care information, liaising with other relevant health and social care professionals for Independent Review process. Take a lead for Continuing Healthcare in Multi-disciplinary case conferences for clients requiring specialist services or request/applications for funding from Continuing Healthcare. In these instances the postholder will be required to analyse situations and use their judgement and specialist knowledge to determine an appropriate response. The postholder will also be required to use knowledge and experience in the analysis of care needs. Take a lead role for health in developing specialist services for clients with enduring and complex health and social care needs.
