Summary


As a member of Adult Community Healthcare Team, the post holder will be providing nursing care and assessment for designated patients.
The overall aims of the Adult Community Healthcare Team are to avoid inappropriate admission into the acute service, facilitate timely discharge into the community and enable individuals to be cared in their home environment.

The post holder will be appropriately trained and supervised in various skills to work independently supporting the District nursing service

The Southern Adult Community Health Care Team (ACHT) are currently based at the Chalfont's Hospital and are delighted to announce a new opportunity for a full time Band 3 Community Health Care Assistant.

The post will involve working in the community, supporting the community nurses and promoting independence in the patient’s home environment.

This is an exciting and innovative service. Candidates to be experienced carers and be able to demonstrate having experience or knowledge of supporting patients in their own homes. You will have excellent communication skills and the ability to work with a range of health professionals and informal carers. The ideal candidate will be flexible and adaptable, being able to respond to need at short notice.

In order to respond to the healthcare needs of the local population, the services covers a 7 days a week rota which therefore includes weekends and bank holidays

Ideally the candidate will have experience and must hold a NVQ level 3 or equivalent in Health and Social Care .

The ACHT reflects an integrated approach that has been designed in response to the changing needs from the local population, with a special emphasis being placed on the proactive vision of care for patients in the community setting.
