Summary


• To work as a named nurse for men with locally advanced and metastatic prostate cancer, providing support and information to men and their families. Across Hampshire Hospitals NHS Foundation Trust.


To work autonomously to deliver specialist clinical care appropriate to the needs of patients with locally advanced and metastatic prostate cancer.
• This will include clinical assessment, planning, and initiating care and treatment, monitoring of disease progression and efficacy of treatment and the proactive management of treatment and disease related side effects. Within independent nurse led clinic review and telephone support.
• To act as a key link between the multidisciplinary team and the patient, ensuring communication across professional boundaries. Facilitate the transition through to clinical oncological / urology care at a timely and appropriate time and continue the named nurse role.
• Provide evidence-based care for a caseload of patients alongside the clinical oncologist.
• To provide support to the wider prostate cancer CNS team during planned leave and sickness.
