Summary
To be responsible for providing a culturally sensitive, and evidence based Haemoglobinopathy service in the community setting To be able to communicate highly sensitive and complex information to professionals, patients and to those who are important to them, often as a lone worker in the community. This communication could be about diagnosis, treatment or prognosis. Lead in providing a person-centred holistic approach to assess the mental, physical, psychological and social wellbeing abilities and needs of the person, their families and carers. To be responsible for providing advice and support to all patients in the borough with a haemoglobinopathy and their families. o ensure that patients and their families are fully informed and supported by providing clinical and support.
