Summary


Exciting opportunity for a band 5 Staff Nurse to join our MDT team on Meadowview intermediate care unit based in Queen Mary's Hospital Sidcup. The role of the Staff Nurse is to bring together different professionals to implement a timely and person centred assessment to determine appropriate intervention for a seamless rehabilitation journey within the unit. This involves partnership working across agencies at the interfaces of primary and secondary health and social care.

As this is a nurse led unit, you will be responsible for supporting the coordination of the day to day care of the patients, the role combines supporting the nurse in charge in management for the junior staff within your delegated team and ensure mechanisms and processes are in place and maintained, you will also act as a source of specialist and expert nursing knowledge and provide support and advice to the wider team.

Main duties of the job
• Provide a high level of support to the staff working on the Unit to ensure effective multi-disciplinary working.
• Participation in the team, working with patients and carers effectively to ensure timely discharge from the service.
• Support in the fostering of excellent relationships, clear protocols, procedures and documentation between nurses in the Unit and other nurses – in the community, on the wards, in A&E, Urgent Care Centre and the Acute Medical Unit.
• Develop and maintain links with community nurses developing clear protocols, procedures and documentation – ensuring the patient is kept at the centre of the care plan.
• High standards of Nursing documentation and communication within the team and with other professional referrals, building on best practice.
• Support the development of basic and advanced nursing skills in the Intermediate Care service, providing clear framework for competency assessments.

Detailed job description and main responsibilities

The Staff Nurse will play a pivotal role in working as part of the multi-disciplinary team, including health and social services colleagues to meet the needs of clients referred to Meadowview Unit
• To be able to contribute to the development of systems and team structures in order to improve service delivery.
• To support with the clinical management in the Unit whilst reporting to the lead nurse and unit manager/Matron.
• To contribute to the development and setting of standards and priorities, promoting quality management standards and the effective implementation and monitoring of standards, policies and procedures.
• Ensures the clinical effectiveness of the nursing team and have the ability to set standards and ensure the quality of care through audit and benchmarking.
• To develop and evaluate the nursing component of Intermediate Care Service, focusing on the hospital/community interface, in conjunction with the lead nurse , Unit Manager and Matron of the Service.
• To ensure that reports and legal records of patient care are maintained and audited regularly.
• To contribute in conjunction with the lead nurse and Unit Manager to the development, maintenance and updating of management information systems in accordance with Directorate procedures.
• To work with the Nurse in Charge, Matron and Unit Manager in supporting the Trust and Strategic Health Authorities to ensure that plans are improving access and services across the board for patients.
• To attend meetings as required, staff meetings, multi-disciplinary meetings and Trust Meetings
• Flexibly plan and organise own time.
• Be flexible in the undertaking of any other duties as requested by line managers to meet the changing needs of the service.
• Identifies opportunities for change and initiates planned change.
• Ability to monitor the change process, ensuring that changes are planned, recorded, implemented and evaluated appropriately.
• Participate in audits.
