ASW - Social Worker ( 2+ years post qualification experience) - Independence & Access
Description
We are seeking an experienced ASW - Social Worker to join our team. This position requires a minimum of 2 years post-qualification experience and focuses on providing support and services to individuals needing assistance in gaining independence and achieving their goals. The ASW - Social Worker will play a key role in creating and implementing strategic plans to empower our clients and promote their overall well-being.
Responsibilities
Conduct assessments and develop individualized care plans based on the unique needs and goals of each client
Provide counseling and support to individuals, families, and groups
Assist clients in accessing community resources and services, such as housing, healthcare, and employment opportunities
Advocate for the rights and best interests of clients, ensuring their voices are heard and their needs are met
Collaborate with other professionals, including healthcare providers, psychologists, and legal services, to coordinate comprehensive care
Monitor and evaluate the progress of clients, adjusting care plans as needed
Maintain accurate and up-to-date documentation of all client interactions and interventions
Requirements
A minimum of 2 years post-qualification experience as a Social Worker
Master's degree in Social Work or related field
Certification and licensure as a social worker within the state
Understanding of human behavior and the impact of social, cultural, and economic factors on individual well-being
Knowledge of community resources and ability to connect clients with appropriate services
Strong interpersonal and communication skills, with the ability to establish rapport and build relationships with diverse populations
Ability to work independently and as part of a team, with excellent time management and organizational skills.
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:30
Pay Per Hour: £24.94
