We are looking for proactive clinicians to join our urgent response service, providing urgent care to unwell patients in their own or care homes within 2 hours of a healthcare professional referral and to optimise the management of patients living with complex long-term conditions in the community.

The team works across Medway supporting our primary care, community services and the ambulance trust in keeping patients safe and well at home.

The successful applicant will have experience of being an autonomous practitioner within urgent and pre-emptive care delivery in community settings.

• To work as part of the multi-disciplinary urgent response service to support unwell patients in domiciliary and care homes. To undertake face-to-face assessment, diagnosis and treatment for community patients presenting as unwell to prevent hospital admission where appropriate and keep patients safe within their own homes.
• Additionally Advanced Community Practitioners case manage individuals whose condition is impacting their quality of life, they may be considered to be frail elderly and/or living with a long term condition(s). Following assessment facilitating their access to appropriate services both internal and external to the organisation. Evaluating any associated outcomes in collaboration with their GP and other health professionals.
• The service operates from 0700-1900hrs 7 days a week and the post holders are required to work shifts, either 7.5 hours or 12 hours.

The small print

• To work as a member of a multidisciplinary team providing clinical care for patients within research and evidence-based guidelines. The team comprises of advanced clinical practitioners, paramedics, phlebotomists, therapists, care managers and social care providers.
• To initiate, negotiate and ensure the provision of the necessary health services to the patient to prevent an unnecessary hospital admission for people in nursing /residential homes and their own homes and facilitate the provision of any required care at home to allow an appropriate timely discharge from hospital, including liaising and referral to other specialist/integrated teams.
• As an autonomous practitioner, working as an integral member of the long term condition teams and the community nursing service. The post holder will be expected to take a lead role in the advanced clinical assessment, management and treatment of patients of adults who require case management in a community setting (including residential/nursing homes), in the Medway area.
• To actively promote self-management which enables the patient to proactively manage their long term condition and know how to confidently manage or respond to any expected exacerbation or deterioration in their condition.
• To work collaboratively with the patient and or family/carer in the use of technology including tele-technology in the management of their condition.
• To work closely with other members of the team, provider services, health and social care partners and GP utilising tools such as Risk Stratification to identify high risk patients providing an holistic assessment and assurance that they are able to access the relevant services.
• Act as a central point of access for complex patients who are not currently known to services in Medway Community Healthcare.


1. Communication and relationship skills
• Communicate effectively with patients, carers, professionals and own team members using a wide range of verbal and non-verbal communication tools within your established counselling and negotiation skills to manage differing professional opinions, share assessment findings, explain highly complex and/or highly sensitive and difficult information, agree treatment plans, progress rehabilitation, give advice and teach. This will include dialogue with patients with communication and comprehension difficulties and those who may be unable to accept a diagnosis/prognosis.
• Liaise with Provider Services staff and primary care staff, including the patient’s GP, Social Services and other mainstream and voluntary groups/services as necessary.
• Gain valid informed consent and work within a legal framework with patients who lack the capacity to consent to treatment.
• Work collaboratively with services in Medway Community Healthcare and social care teams to develop and implement patient partnership initiatives and improve patient communication/information systems.
• Follow the Complaints Policy and treat any complaint from a client or carer seriously, reporting it to the manager and if required assisting with the initial investigation into the complaint at the informal stage.




1. Knowledge, training and experience
• Recognised degree/diploma in Nursing/Paramedic Science.
• Registered Nurse on Part 1 of the Nursing Midwifery Council register/ Hold current registration as a Paramedic with the Health and Care Professions Council.
• Specialised post-graduate qualification, i.e. in elderly care, chronic disease management, rehabilitation or teaching.
• Prescribing qualification (Level 5) or willingness to undertake within one year of appointment.
• Educated to Masters level or willingness to undertake relevant masters qualification
• Substantial postgraduate accumulative experience in the fields of elderly/rehabilitation/community/intermediate care/long term conditions.
• Proven experience in an autonomous practitioner role is essential.
• Evidence of specialist knowledge in either minor illness management, community nursing or clinical assessment as an advanced practitioner is desirable.
• Experience of management skills to enable consideration of access, cost, value and quality when making health care decisions.
• Knowledge of current issues within Urgent Care, Planned Care, long-term conditions and health and social care in general.
• A working knowledge of the admission avoidance ethos and facilitation of timely discharge.
• A sound knowledge of Clinical Governance and risk management.
• Intermediate IT (Information Technology) skills (Windows, Internet, Outlook, Excel).
• Presentation and teaching skills.
• Minor Illness and triage course, or willingness to undertake is desirable.
• Recognised, recent Intravenous therapy and cannulation training, or willingness to undertake.


1. Analytical and judgement skills
• Initiate own investigations and diagnostics and interpret results in order to take appropriate action to manage the patient care and refer to appropriate services where necessary.
• Provide advanced assessment of patients presenting with a range of complex clinical conditions in order to diagnose, recommend and advise referring clinical colleagues of the appropriate care.
• Identify and manage risk in relation to patients with complex conditions.
• Regularly assess and act on the views of the patients, relatives and staff about the standards of care given, ensuring that comments are dealt with promptly and courteously, upholding the role of patient advocate at all times.




1. Planning and organisational skills
• Report on patients’ conditions to appropriate medical practitioner (Primary Care Physician, GP or MedOCC) or consultant practitioner (admission avoidance, stroke, respiratory, cardiology and muscular-skeletal) as necessary and ensure prescribed treatments are carried out.
• Assist with team management to inform and manage change and ensure that all care is managed in a safe and timely manner.
• Responsible for proactively monitoring and managing a designated caseload of patients with complex short and long term conditions.
• Initiate and organise the necessary services to the patient to prevent an unnecessary hospital admission.
• Facilitate the provision of any required care at home to allow an appropriate timely discharge from hospital, including liaising and referral to other specialist teams.
• Facilitate the development of new skills and knowledge within the Integrated team to support advancement in clinical care provision in line with developments in health care provision.


1. Physical skills
• Using advanced clinical skills undertake the physical examination of patients in order to assess, evaluate, plan, and implement programmes of care to ensure effective high quality, evidence based health care and health promotion in accordance with best practice standards. This will include observation skills and the dexterity to perform skills including (but not exhaustive) such as cannulation, administration of intravenous injections, syringe pumps and infusions, the insertion of catheters, application of primary wound dressings and the taking of blood for blood tests when appropriate.


1. Responsibility for patient / client care
• Initiate own autonomous clinical assessment (including telephone triage) using a range of advanced skills and expert knowledge of intermediate care, of patients presenting with complex and undiagnosed conditions, facilitating diagnosis, specialist advice, treatment, referral, review or discharge.
• Initiate and review care programmes appropriate to the needs of individual patients in a community setting, including patients within residential and nursing homes.
• Develop and maintain regular contact with staff and residents in care homes across Medway, as appropriate, undertaking clinical reviews of residents’ treatment plans and current clinical management strategies, conducting individual medication reviews, whilst being supported by colleagues from the Intermediate Care services, Integrated Teams or other specialist teams where appropriate.
• Prescribe medication as an Independent Prescriber and take responsibility for prescribing in line with the Nurse Prescribing Formulary, NHS Medway and national policy/guidelines, or prescribe, administer and dispense medications against a ‘Patient Group Direction’ whilst undertaking Independent Prescribing training.
• Advise and support patients with regard to healthy lifestyles, health promotion and disease prevention.
• Recognise and report environmental health problems affecting patients and provide health protection interventions that promote health and safe environments for patients and staff.
• Maintain clinical and professional competence by portfolio evidence that shows an up to date knowledge of nursing, therapy, health, social and integrated working care trends and policy.
• Maintain a professional manner and promote a positive image of your employing organisation.
• Actively participate in regular clinical supervision as supervisor and supervisee
• Provide training to support the development of less experienced staff.
• Participate in the appraisal programme and identify training needs, ensuring that statutory, mandatory and professional training and registration is completed and updated in a timely manner.
• Attend, activate and participate in staff meetings.


1. Responsibility for policy and service development implementation
• In conjunction with the operational and professional lead, be responsible for the development, implementation and evaluation of intermediate care pathways, protocols and policy across Medway, including proposing service development improvements where the care pathway crosses those of other community services
• Identify and progress practice development to improve patient care standards and outcomes.


1. Responsibilities for financial and physical resources
• Be responsible for the setting up of medical/surgical equipment for safe use by the patient.
