Summary


Thisisafixedtermfulltimepost whichhasarisenasaresultofmaternitycover.Itis basedpredominantlyin Andover, although with the ability to work cross site and provide cover to Basingstoke and Winchester if needed, to ensure service needs are met. The post involves working within the purpose built Countess of Brecknock Hospice, providingspecialist palliativecaretotheteninpatients.Italso includesopportunitiesfor offering outpatient appointments and seeing patients in the community in their own homes. There is a 1:6 non-resident on call providing remote telephone advice out of hours to the HHFT palliative care inpatients and face to face support to the Winchester Hospice site at weekends.





To work with the Consultants in Palliative Medicine to deliver high quality palliative care.

Undertake medical assessment, diagnosis and management of patients in hospital / community / hospice.

Initiate appropriate medical care and prescribe appropriately in line with HHFT policies and guidelines.

Assess new planned and emergency admission patients and work with MDT to initiate appropriate investigations and treatment.

Any other medical duties in emergency situations.

Communication with patient’s relatives and carers regarding clinical condition and management plan. To be involved in family meetings as required.

To work as part of the MDT and communicate clearly with colleagues. To attend weekly Specialist Palliative Care MDT meeting.

Communicate unexpected changes in patients’ condition to Consultant in charge and, when required, to on call medical staff.

Record medical assessment and examination daily on electronic patient database.

HHFT is unique in having 10 hospice beds on each of the 3 geographical sites.

St Michael’s Hospice (SMH) is an independent hospice in the grounds of BNH. SMH provides inpatient, day care, hospice at home and community services to the population of Basingstoke and Deane. The location of the hospice facilitates a close working relationship with HHFT and seamless clinical care between the two organisations. In Basingstoke one integrated weekly multidisciplinary team meeting takes place for all parts of the HHFT and SMH services to support this.

Winchester Hospice (WH) opened in September 2021 in the grounds of RHCH. This has 10 beds on 2 floors. Palliative care outpatient clinics and a domiciliary home care service run from here, day services are planned to open in the coming months.

The Countess of Brecknock Hospice (COBH) in Andover is a newly rebuilt 10 bed hospice with community, hospice at home, domiciliary home care, outpatient and day care services.

As part of Palliative Care Multi-disciplinary Team, the post holder will deliver high quality professional care to patients and relatives within the Hospital, Community and Hospice as needed. He/she will work closely with the Clinical Team to ensure we offer the highest standards of care, developing services to meet the changing needs of our patients.
