Summary


The NHS Health Check team is looking for an enthusiastic and flexible Health Check Adviser to work across Kent delivering NHS Health Checks to eligible individuals aged between 40 and 74.
The Health Check Service supports individuals in Kent by offering a person-centred approach to raising awareness of their risk of developing cardiovascular disease and the impact of their existing health and lifestyle behaviours. We then support them to reduce their risk through onward referral for further investigation and/or lifestyle support.
The service is working to reduce the impact of health inequalities by offering Health Checks in a wide variety of settings around the county.
We provide full on-the-job training so specific experience of delivering Health Checks is not required, though some clinical knowledge/experience is beneficial.

• To deliver high quality Health Checks to eligible individuals at clinics, community venues, workplaces and outreach events across Kent.
• To take and record accurate physical measurements (including using fingertip blood tests) to calculate an individual’s risk of developing cardiovascular disease
• To undertake meaningful conversations to communicate an individual’s risk and to encourage lifestyle changes using behaviour change methodologies
• To refer individuals for further investigation and/or lifestyle support as appropriate
• To undertake administrative tasks in relation to our targeted invitation process, including mail merges, telephone calls to patients and accurate record keeping
