Summary
Main duties Main duties of the job Responsible for implementing processes for the effective management of patients with long-term conditions using evidence-based practice. The post holder will ensure that healthcare assistant services such as dressings, annual reviews, phlebotomy, injections under PSD and NHS healthcare checks are effectively delivered to the entitled patient population. Responsible for health promotion, chronic disease management, as well as actively supporting the clinical team in the delivery of clinical policy and procedure.
