Summary


A Monday-Friday 8am-4pm working pattern is just one of the attractions this opportunity provides. We are looking to recruit a Band 7 Community Heart Function Nurse Specialist based Upton Hospital in Slough to cover our Windsor and Maidenhead caseload.

You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for case management of patients with heart failure in a community setting.

You will be responsible for ensuring high quality evidence based care for patients with heart failure, their families and carers in the community.
• Run community-based Heart Failure clinics in various community settings
• Work flexibly in response to service need across multiple sites
• Take responsibility for case management of patients with heart failure in the community setting
• Supporting colleagues in the delivery of compassionate, safe and structured care
• Make differential diagnosis within level of competence
• Plan and provide treatment in collaboration with others as appropriate

1. Recent Clinical Experience within the speciality
2. Advanced Physical assessment skills
3. Demonstrate effective Chronic Disease management
