Summary
Following a successful contract win to deliver care across the South East and South West Sheffield area, we`re now looking to recruit an experienced Trusted Assessor/Reviewer to be part of our team!
We`ve been in the care business a long while, over 50 years, operating in the North East, Yorkshire, and Humberside, and we`re passionate about delivering the highest quality care and support for people living in their own homes.
The role of the Trusted Assessor will work closely with other health professionals and colleagues, delivering individualised support based on what each person needs, with continuous review of how needs are being met to maximise independence.
Key Tasks
Empower the customer and support them to achieve greater independence and engagement in the community
To build relationships with adults, their families and informal network supports.
To ensure changes made in full consultation with the person in receipt of care and workers in adult social care
Support to build capacity and focus on core statutory responsibilities such as safeguarding, DoLS, Court of Protection and formal review of support plans
To understand the roles and accountabilities of other agencies and professionals to refer, liaise, consult and work effectively in order to meet identified needs
Contribute to the reviews of support plans as required, ensuring the right level of support is delivered only for as long as is needed, advising the person and their carers of alternative options as appropriate.
Skills & experience
Experience of working within a local authority Adult Social Care, Care Provider, or NHS Health Care
Level 3 in Health and Social Care
Experience of working within a multi-disciplinary team
Experience of completing reviews, and assessments using evidence based decision making.
