Summary

We welcome applicants to this fixed term 1 year consultant post to cover a career break.

The Medicine for the Elderly department provides facilities for both acute and rehabilitation patients along with appropriate multi-disciplinary support team. We have 2 wards at RPH (ward 17 and acute frailty assessment unit AFAU) and 3 wards at Chorley hospital (Brindle, Rookwood A and Rookwood B).

Medicine for the Elderly has expanded recently with the establishment of an acute frailty assessment unit (AFAU) at Preston, surgical liaison services (general surgery, vascular surgery and major trauma), hot clinics in the community and 4 new consultant appointments within the last 3 years. We have good relationships with our local CCG and with the local GPs: the community hot clinic is housed at a local GP practice.

The team has been pioneering in screening for delirium and dementia in the elderly as well as providing comprehensive geriatric assessments. We were BMJ finalists in 2016 in the field of innovation.

The department would offer any newly appointed consultant the opportunity of mentorship. This would be offered by the existing consultant geriatricians who would guide and advise the new appointee. It would be expected that any new consultant would have an annual appraisal with a trained appraiser and the Trust would support this process fully.

You will be expected to:
• Provide direct input to an elderly medicine ward, assuming a continuous commitment for the care of ward inpatients, even if employed less than full-time, ensuring adequate arrangements are made for leave and off-duty periods. In-patient numbers are typically 16 care of the elderly patients and up to 5 outlying general medicine patients. You will provide oversight and clinical supervision for junior doctors attached to your ward.
• Deliver care alongside existing geriatricians and surgical teams to the Major Trauma and vascular surgery services, ensuring our elderly surgical patients receive the best possible treatment in order to optimise patient outcome.
• Carry out a general geriatrics & falls clinic. Typical clinic numbers are 6 new and 2 follow up patients.
• Participate in the General (Internal) Medicine consultant 1 in 18 on-call rota at Royal Preston Hospital, including conducting the ‘Post-Take’ ward round.
• Promote high professional standards of clinical care for patients, both through direct patient care and by the maintenance of continuing professional development.
• Participate in clinical governance meetings.

Lancashire Teaching Hospitals NHS Foundation Trusts is one of the major acute Trusts in the North West with all of the usual specialties on site as well as being a Tertiary Centre for Lancashire and South Cumbria in the departments of Respiratory medicine, Neurosciences, Oncology, Plastic Surgery, Burns, Renal Medicine, Vascular surgery and Trauma medicine. Undergraduate medical education forms a core part of the Trust’s educational roles, which the appointee will be expected to be involved in.

Lancashire is an attractive area to live and has many facilities available locally. The cities of Manchester and Liverpool are within easy reach. Recreational facilities are many and varied, with the Lake District, Ribble Valley, Yorkshire Dales and Fylde Coast nearby.

Any person who is unable to work full-time for personal reasons will be eligible to be considered for the post and if appointed the job plan will be discussed on a personal basis in discussion with colleagues.
