Swaffham Community Hospital is a 18 bedded unit providing rehabilitation to a wide group of patients with varying complexities of needs including palliative care. Our patients come to us following medical, surgical or orthopaedic treatment or if appropriate, directly from their own homes.
Good opportunities exist for further training and development. You will receive support/development through in service training, regular appraisals and clinical supervision. This post involves rotation between day and night duties. The Trust supports flexible working.I
• Be responsible for the delivery and co-ordination of care to a named group of patients
• Work effectively as a member of the multi-disciplinary team.
• Work proactively within the team to achieve unit and trust goals and objectives.
• Work with and promote recognised policies, protocols and guidelines

• Current UK NMC registration, level 1.
• Working towards Degree (desirable).
• Experience of working within a multidisciplinary team.
• Experience of working with patients with multiple pathologies and developing and delivering care plans designed to meet their needs.
• Appropriate short clinical skills e.g. venepuncture, catheter insertion.
• Excellent verbal, written and interpersonal communication skills.
• Basic computer skills.
• Ability to maintain and improve standards of care.
• Experience of audits.
• Up to date knowledge of current clinical/professional issues.
• Understanding or resource management.
• Knowledge and understanding of learner needs.
• Able to communicate effectively in written and verbal English Language.
• Evidence of relevant experience and development.
• Willing to undertake appropriate training (desirable).
• Self-awareness.
• Flexible/positive approach to work.
• Enthusiastic and able to work under pressure.
• Able to work collaboratively as part of a team.
