Summary


An exciting opportunity is available to join the Children's Community Nursing Team.

The post holder will work as part of an established Children’s Community Team and be responsible for the assessment, planning, implementation and evaluation of evidence based programmes of care for the child, young person and their family within the community environment. The post holder will also assist in the management and organisation of nursing work in the community setting.

You will need to be an experienced children’s nurse/nurse associate and be able to demonstrate evidence of recent Paediatric experience at Band 5 or Qualified Nursing Associate. You will need to have the ability to work independently, flexibly, and be responsible for implementing care and managing a caseload. A good level of clinical skills are required to deliver care to families across multiple medical conditions. Excellent communication skills and the ability to work effectively with families and colleagues is essential.

As a member of the CCN team you will be involved in both service developments and delivery of family centred care which we continue to provide with motivation, passion and enthusiasm.

To be responsible for the assessment, planning, implementation and evaluation of evidence based programmes of care for the child, young person and their family within the community environment.

Provide care at home or within a community setting without supervision to facilitate the prevention of admission or reduction in hospital stays.

Liaise directly with the acute ward setting, the multidisciplinary team and other agencies to facilitate seamless high quality care to the child and family at home and in the wider community.
