Summary

The Pathway 1 team based at St Mary’s Hospital work closely with the acute hospital at Imperial Healthcare NHS Trust, Westminster and Kensington & Chelsea Adult Social Care teams, the Community Independence Service and other CLCH community teams to discharge patients home safely with community support.

We work with colleagues to coordinate discharge plans, ensure safe environments, equipment, care and support plans are in place.



This is a great role for those with sound clinical assessment, problem solving and great interpersonal and negotiation skills. A background in older people’s care, frailty and/or community services would be ideal.

As a nurse or AHP in-reaching into the acute integrated discharge team, you will also be a touchpoint to promote community and acute relationships and communication.

The funding for this post is secured until the 31/03/2025 upon which we hope the funding will be made permanent depending on the success of the Pathway 1 work and associated business cases. We will therefore also consider those wishing for Secondment opportunities.

• To demonstrate discretion and respect when communicating with patients, colleagues and other agencies.
• To use tact and diplomacy to provide and communicate complex, sensitive information where motivational negotiating empathetic and reassurance skills are needed, where barriers exist to understanding, maintaining confidentiality within Trust Policies.
