Summary


Dorset HealthCare are looking for a Team Leader to join our dynamic Integrated Community Rehab Team based in Dorchester.

As part of the Dorchester ICRT, you will be part of the West Dorset Cluster, which has a focus on both Discharge to Assess and Admission Avoidance including Urgent Community Response and proactive management to optimise independence. As a Team Leader you will lead and manage the ICRT Team and support developments both locally with the PCN and Trust wide, enabling staff within the team to fulfil their roles and develop the service.

As the Team Lead you will need excellent communication, proven leadership experience, be confident in leading across disciplines and motivational skills, experience of managing change and working across the wider health and social care system to drive service development.

The role is a full-time Monday to Friday, we can be flexible and would consider condensed hours. You will work either or both Saturday and Sunday per 6-week roster period at enhanced pay! You will also get a day off in lieu of the working week.

To provide leadership to a multidisciplinary Integrated Community
Rehabilitation Team to deliver patient centred care to people requiring
community based physical health rehabilitation and health management.


Responsible for ensuring the delivery of high-quality person-centred
services with a focus on cluster based, integrated care which always
considers peoples safety, privacy and dignity.


Day to day management and leadership of therapy and nursing
practitioners and other staff in the multi-disciplinary team.

To provide senior clinical advice, leadership and support to the team

Day to day clinical management of the service, including rotas and clinical cover day to day across the service.

Attendance and participation during meetings where clinical leadership and expertise is required.

Supervision of staff within the service.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
