PERSONAL NAVIGATOR X2, CHANGING FUTURES, NOTTINGHAM, 2 X 37 HPW, 12-MONTHS TEMPORARY CONTRACT, FRWK08 £24,156 - £26,076 FTE
We exist to improve the lives of people with Severe Multiple Disadvantage (SMD) in Nottingham City and deliver our work through a partnership of local agencies, of which Framework is the lead.
Clients of Changing Futures will be experiencing at least three out of the following Five issues: homelessness, substance use, offending behaviour, domestic violence or mental ill health.
We are recruiting for: Changing Futures navigator x 2 (Full time 37 hours per week – 12 Months temporary contract)
The main purpose of the job will be to coordinate delivery of services to individual clients and directly support a client caseload. The successful candidate will also work with Clients to create Support Plans and liaise with referring & partner provider agencies.
This group of clients’ present challenges, therefore the team and its members need to be resilient, creative, flexible, patient, and persistent and, where required, assertive in their approach.
We are looking for a candidate who can demonstrate:
• Experience of working with vulnerable people, particularly those with SMD
• Understanding of partnership working with external agencies
• Understanding of the range of services available to vulnerable people
• Understanding of the issues faced by people who are affected by: homelessness, substance use, mental ill health, domestic violence and offending behaviour
• Ability to accurately plan and record work with beneficiaries
• Commitment with tolerance and flexibility.
• Excellent communication, liaison and negotiation skills
