Summary


We have an exciting opportunity for an Integrated Community Manager to join our team leading the Rapid Therapy, Discharge to Assess Nursing teams and support workers within the Derby City locality.

The role will include leading a community team made up of physiotherapists, occupational therapists, nurses, support workers and admin staff who work together to provide Urgent Community Response and Discharge to Assess Services in Derby.

We are also currently formally integrating these teams with the Derby City Council services who also provide discharge to assess and urgent community response to create a new Community First service. The successful applicant will have the opportunity to contribute to and lead on aspects of this work and be a key part of this exciting development.

Although the post is substantive, we would welcome applications from people looking for a secondment opportunity.





As well as providing inspiring and supportive day to day operational management, there will also be lots of scope to participate in service development and transformational change, particularly around the integration of DCHS services with Derby City Council.

The ICM role will be pivotal in working with the Integrated Discharge Team based at Royal Derby Hospital and with the Acute Home Visiting Services based out of the Local Navigation Hub.

The post is open to qualified Nurses, Occupational Therapists or Physiotherapists.

It is an ideal opportunity for an experienced integrated team leader to make a first step up to ICM level or possibly an already established ICM looking to broaden their skills and experience.

The post holder will work very closely with the managers who cover Home First (the City Council discharge to assess teams) and Hospital to Home (the hospital social work team for Derby City) and the Integrated Community Managers who manage the core and rapid services across Derby City, as well as county-wide, with peers who meet regularly for support and project development work.

Mentoring and leadership development will be supported.

You will be highly motivated, enthusiastic, have excellent communication and leadership skills and have a current working knowledge of the integrated care agenda.

Your role will be working within DCHS to ensure our staff are engaged and motivated by the changes and that you work in close liaison with our commissioning, social care, primary, secondary care colleagues and other partners to redesign pathways of care.

To be responsible to the Head of Community First for the delivery of high quality, patient centred, integrated, cost effective care across the locality by having managerial responsibility across community nursing, enhanced community practitioners and community therapy.

You will be accountable for quality and performance management, contract management, marketing and service improvement of all the community teams within your locality.

Have lead responsibilities within the community-based locality team for the provision, co-ordination and development on an integrated community-based team approach, working with the head of Service and other members of the locality team to promote and foster partnership working with key stakeholders.

In conjunction with the Head of Service and wider DCHS team, develop business cases and subsequent project plans.

Manage effective and efficient services within an agreed budget

Contribute to the development and implementation of service strategies and plans within the Division and across DCHS.

Manage and provide clinical leadership to the Integrated Community Team Leaders and across the community teams
