Summary


This is an exciting opportunity to work at John Hunter Clinic as a BBV Health Adviser and Pathway Coordinator for patients with reactive BBV result on testing in the Emergency Department (ED) within CWFT. The majority of these will be unexpected diagnoses so patients may require advice and support to encourage them to enter care.

The post holder will work with the health adviser team and clinical leads to ensure patients are receiving their results efficiently and they will be expected to provide health education, advice and support to patients following their diagnosis along the patient pathway into care.


Job Summary;

• Clinical Practice


• Provide information and health education for clients diagnosed with BBV infections, including HIV providing support and onward referral as required


• To manage the process of partner notification/management for these clients in order to encourage, with the client’s consent, the attendance of all potentially infected sexual contacts to a Sexual Health clinic


• To manage the patient pathway for all patients with reactive blood borne results who were tested in the ED department.


• To develop and maintain supportive and effective relationships with the Hepatology team in the hospital.


• To exchange relevant information about clients and contacts with other Sexual Health clinics as necessary.


• To take sexual histories from clients as appropriate and refer on to relevant individuals in the multi professional team as part of the care pathway.


• Provide HIV post-test discussion/support for those clients who have a reactive or positive HIV test result, ensuring they are linked in to HIV outpatient services and facilitating onward referral where necessary to other appropriate services.


• Where appropriate, supporting partners, significant others, and immediate family of those newly diagnosed with HIV, including facilitating partner notification, testing and disclosure.


• To be able to perform Point of Care HIV testing in accordance with Directorate guidelines.


• To be able to perform phlebotomy as required in accordance with Trust and Departmental guidelines


• Maintain accurate client records, ensuring all entries are dated, signed and designation recorded as in accordance with the Trusts policies and procedures

Data and Reporting


• Data collection and regular reporting of the linkage into clinics for patients with a positive BBV test.


• To be responsible for tracking monitoring/treatment pathway for BBV patients.


• Collate reports and updates for the clinical leads when requested.


• Disseminate reports relating to activity to the clinical lead, Hepatology team and Senior Health Advisor as necessary.


• Provide up to date and accurate data to complete required monthly, quarterly and annual reports.

Education and Development


• Initiate and participate in Clinical Audit, service development and appropriate research projects with particular regard to Partner Notification outcomes


• Partake in the teaching of other healthcare professionals, students and new members of staff, e.g. medical students, nurses, doctors, social workers


• Participate in annual appraisals ensuring own personal and professional development is Maintained


• 0 Professional Responsibilities


• To be aware of Clinical Governance and complete mandatory Clinical Governance training


• Attend meetings of academic and clinical relevance held within the Department. To keep abreast of the latest research, procedures and treatments pertaining BBV including HIV


• Participate in clinical and managerial supervision sessions


• To attend the sexual health adviser team meetings


• To develop and maintain effective communication and liaison with other members of the Multi-professional team

Training and personal development


• To employ a pro-active approach to good team working, including being able to cover the responsibilities of colleagues in similar roles within the health advisor team and across site where needed.
• To collaborate with clinical lead, senior health advisor and Hepatology teams to promote service improvement and development.
• To attend relevant meetings and represent data quality as requested.


• To ensure skills and knowledge are up-to-date and relevant to the role.
• To follow relevant Trust policies and professional codes and to maintain registration where this is a requirement of the role.
• To participate in planned training events and to attend all mandatory training sessions as requested.
