In this role, you'll collaborate closely with our Lead Dietitian and team members to provide excellent dietary services across various settings, including patients' homes, community hospitals, care homes, and clinics. You'll have the chance to work with a diverse range of conditions, from nutrition support to managing coeliac disease, gastroenterology issues, weight management, diabetes, and more.
At Surrey Downs Health and Care, we offer a hybrid working model, granting you the flexibility to work remotely or at multiple sites throughout the borough. We're committed to your growth, providing opportunities for student training and professional development. Expect regular supervision and continuous professional development opportunities to enhance your skills and expertise.
This full-time position, Monday to Friday, comprises 37.5 hours per week. We're open to discussing part-time or job share arrangements. Join us in making a meaningful difference in the lives of our community members by ensuring they receive the highest standard of dietetic care and support.

As a key member of our team, you'll be responsible for providing excellent dietetic services to community patients, their families, and carers throughout the Surrey Downs Health and Care locality. Your main duties will include:

• Delivering comprehensive and high-quality dietetic services in line with professional standards and clinical governance requirements, collaborating closely with all members of the dietetic department.
• Effectively managing your workload and time to ensure you meet deadlines, ensuring efficient and timely service delivery.
• Making informed recommendations to General Practitioners (GPs) regarding the appropriate prescribing of oral nutrition supplements and enteral feeds, adhering to local policies and guidance on prescribing practices.
• Supporting the implementation of National Institute for Health and Care Excellence (NICE) and National Service Framework (NSF) guidance, as well as local care pathways to enhance patient care outcomes.
• Providing supervision and guidance to student Dietitians during their placements, fostering their professional development and growth within the field.

Surrey Downs Health and Care deliver care closer to people’s own communities through our Primary Care Networks, Community Hospitals, Specialist Services and our innovative partnership of local NHS organisations.

Surrey Downs Health and Care has a track record of providing person centered care that goes beyond organisational boundaries to do what is best for the individual. This partnership includes:
• The three GP federations GP Health Partners, Dorking Health Care and Surrey Medical Network representing practices that operate in the Surrey Downs area
• CSH Surrey
• Epsom and St Helier’s University Hospitals NHS Trust
• Surrey Council County

Historically, there have been boundary lines between the organisations that provide care to people in their homes, in GP surgeries and in hospitals, but we have always been united in our mission to provide great care to the people who need us.

It’s on those grounds that the Surrey Downs Health and Care was formed – we want local people to receive the care that they need in the right environment. By bringing together our expertise, we can improve patient care and enable local people to access the right support, care and treatment more easily than ever before.

In bringing this partnership together, we are working to the same set of values that will translate into better care for our residents.
