Applications are invited for this exciting opportunity to cover maternity leave ( part-time 12 month fixed term contract) Band 6 Specialist Nurse in Haemoglobinopathy.

The adult and paediatric services are designated by NHS England as ‘Specialist Haemoglobinopathy Teams’ (SHT) and provide network and outreach care to the following hospitals: Royal Berkshire, Stoke Mandeville/High Wycombe, Wexham Park, Frimley Park, Great Western, Milton Keynes, John Radcliffe, Churchill. The team works to a ‘whole of life’ approach to care, ensuring integration and seamless transition.

There are approximately over 600 children/adults across the Thames Valley region with major haemoglobin disorders (Sickle Cell Disease, Thalassaemia and Rare Inherited Anaemia). The majority of the patients have sickle cell disease, other conditions include, transfusion and non-transfusion dependent thalassaemia, diamond black-fan anaemia and other very rare anaemias.

The successful applicant will have a particular focus on transition for young people across the Network, ensuring patient and family education and a smooth transition into the adult services. Working to an established service and pathway, the post holder will be supported to run telemed/video call nurse led transition appointments with young people and their families, using the ‘Ready, Steady, Go, Hello’ framework. The major goal of this role is to lead and improve transition pathways and processes.

The post holder will work within the broader multidisciplinary team and in conjunction with the adult and paediatric lead nurses to provide: patient review and support; family/carer support; school/higher education liaison and education; GP and community support; local hospital support and education. Along with the clinical team, he/she will be responsible for ensuring the national standards for haemoglobin disorders are met and maintained.

Job profile

The Specialist Nurse will contribute to the care of adult and paediatric patients and families. This role involves hospital and community based working across the Thames Valley Network.

The Specialist Nurse will
• Accept patient referrals to agreed criteria and be identified as the key worker for an agreed patient/client caseload.
• Provide advice, education and support to patients, their carers and other health care professionals.
• Practice clinically in agreed care settings according to approved protocols or guidance.
• Contribute to the smooth running of the designated service.
• Work across departmental and hospital boundaries.
• Liaise with other hospital and community teams along an agreed pathway to support the delivery of patient and family centred seamless care.
• Contribute to the ongoing development of the Haemoglobinopathy service.
• Work in-conjunction with the local service and the regional network using the Lead Nurses for specialist guidance.
• Work towards specifically agreed Key Performance Indicators (KPI) based on local and network service needs.
