Summary


We have an exciting opportunity within the care group to recruit into a pioneering new role.

A support worker to provide support and advocacy

The Activities Coordinator is a new role that will help provide an excellent patient journey. The concept is to stimulate patients both physically and mentally by providing group and individual activities. The role is a full time band 3 position on a permanent basis, part time position can also be discussed.

Activities including Arts and crafts, Music, Relaxation and massage, Bespoke reminiscence packages and Photographs/ Memorabilia

Supporting all patients across the site to aid in their journey. organising activities to support patients their families and staff to boost moral encourage engagement with members of the MDT and provide support to patients to allow a smooth discharge.

working closely with the dementia team enabling dementia patients to maintain/improve their mobility by use of exercise and mobility appliances, according to the patients individual care plan.

Supporting dementia patients with movement and handling within the safe practice guidelines from your mandatory training and in relation to the patient’s manual handling plan.

To carry out tasks which a caring relative might normally carry out. To respond to

the physical, personal, emotional, religious, cultural and spiritual needs of the

individual

· Providing general support in a sensitive and empowering way; ensuring at all times

that privacy, dignity and self esteem are respected.

· To encourage, develop and implement diversional activities, by maintaining the

person’s skills and interests as specified in the Care Plan these b may include crafts

such as art, games, music and/or dancing as well as social outings.









· To facilitate and carry out agreed structured creative communication this may

include reminiscence work, life history albums, reassurance techniques and such, under the guidance and direction from the Team Leader and other Professionals. · To ensure effective, clear channels of communication with team; Nurses, Social Workers; GP’s; Consultants etc. and to work as part of a team with colleagues to cultivate a safe, environment. · To respect and understand the need for confidentiality and professional boundaries. · To regularly report to the Team Leader any problems, concerns, training needs and other relevant issues. · To attend mandatory training and take responsibility for self development by identifying other appropriate courses. · To follow policies and procedures in place for health and safety, administration of medicines etc. · To undergo personal supervision and performance appraisals with line manager
