Crynodeb
Practice Plus Group is building on its extensive experience in Primary Care. We deliver a new model of general practice called Practice Assist. Practice Assist provides additional remote capacity to GP Practices and prisons to give registered patients a full consultation by telephone or video. The clinical team also provide clinical admin support within the prison setting. Practice Assist currently provide support to 7 GP practices and 27 prisons. We are currently looking for Prescribing Pharmacists to join the Practice Assist service. The post holder will lead, and be responsible for the pharmacy support to the PA+ service within Practice Assist. The Prescribing Pharmacist will conduct medication optimisation clinics remotely without the patient being present, and prescribe for patients as deemed clinically appropriate within Practice Plus Group, local and national guidelines. Additionally, carry out long-term conditions review clinics in line with their specialist areas, signposting patients where appropriate. You will also be working within the team management guidelines of roles and responsibilities for NPM. We currently have 2-3 hours per week availability, and this may increase over the coming months.
