Summary


We are looking for an individual who will manage a team of four Band 4 Health Improvement Advisors and provide tailored health improvement advice to our inpatients.

You will work with different departments to seek opportunities to develop, encourage and implement health improvement interventions, including smoking cessation, across our different sites. In particular, you will act as a Smoke Free staff champion for the Trust, using timely, evidence-based practices to help patients stop their tobacco use both during their hospital stay and after discharge. You will engage with community-based smoking cessation services to ensure our patients can continue to receive support after they are discharged from hospital.

The Health Improvement advisor team will contribute to the Trust’s Health Equity Programme and commitment to reducing health inequalities through supporting people to stop smoking, as well as looking at other health improvement opportunities such as weight management and alcohol intake. The team will engage with patients and offer them practical support to change their behaviour to achieve their own choices and goals.

This role requires an ability to manage time and resources effective to ensure the team are meeting Trust’s standards and targets.

Candidates with experience of providing tobacco dependence interventions would be ideally suited to these roles, as would experience working in a hospital setting.

The post holder will work with the Health Improvement Project Lead and manage a team of Health Improvement Advisors, working across adult in-patients wards, to provide individualised health improvement advice support to patients, such as those with a tobacco addiction.

This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking, as well as looking at other health improvement opportunities, such as for weight management and alcohol in-take. It focuses on engaging with patients and offering them practical support to change their behaviour to achieve their own choices and goals.
