Summary


An exciting opportunity has arisen for Registered Nurses to work within the Community Continence and Urology Service on a full time permanent basis.

We are looking for enthusiastic individuals who have an interest in the specialities of continence and urology. You will form part of a well-established team who provide specialist care across CWP neighbourhoods and Care Homes.

The main aspect of the role will be to provide continence assessment and implement and evaluate specialist continence programmes of care. The successful candidate will also participate in the delivery of training patients and their carers, colleagues and the wider Health / Social Care community.

The Community Continence & Urology Service provides specialist care to patients, their families, carers and colleagues across the whole of the West Cheshire Footprint. These patients are seen both within the Community, Care Home and clinic setting.

The successful candidate will join an established team of clinicians, nurses and administrators.

This post is Fixed term until 01/01/2025.

1. To carry out continence assessments, implement and evaluate treatment programmes to patients.

2. Professionally responsible for own caseload.

3. Participate in nurse led continence clinics.

4. Perform clinical skills relating to the post.

5. Training to carers.

6. Responsible for maintaining own confidential caseload notes.

7. Work with external agencies to raise profile of continence promotion.

8. To communicate effectively with relatives, clients, agencies and the multi-disciplinary team.

9. To act as the clients advocate at all time

10. To develop communication strategies to ensure understanding of care programmes and concordance with the treatment plan.

11. To encourage good relationships with other statutory and voluntary services involving collaborative working.

12. To have good communication within the continence team.

13. To effectively communicate sensitive information in relation to, e.g., financial, sexual, interpersonal issues.

14. Responsible for managing and prioritising own caseload.

15. To work within the continence service budget.

16. Participate in risk assessment for patients, carers and staff.

17. Signpost and advise staff as to sources of information as appropriate to their needs/requests.

18. Participate in the audit process in conjunction with the clinical audit department where appropriate.
