Summary


This is a brand new post and an exciting opportunity for an experienced oncology nurse.

Using your skills and expertise in cancer care, you will provide physical and emotional support to patients who have metastatic breast cancer, coordinate their care services, and inform and advise them on clinical and practical issues associated with their illness.

The post will incorporate the Clinical Nurse Specialist (CNS) role dimensions of expert clinical practice, education, leadership, management and research. The post holder will be expected to implement HNAs and work with the team to deliver all aspects of the Cancer Recovery Package.

The post holder will be required to work with clinical, operational, information and administrative staff across all levels of seniority within the acute Trust.

The successful candidate is required to be able demonstrate the Trust values, the responsibilities of the job description and person specification. The post will be based at the Chase Farm Hospital site but will also spend time at the Royal Free Hampstead.



The Cancer Clinical Nurse Specialist will be required:
• To work autonomously and integrate knowledge of cancer and medical treatments into assessment, diagnosis, and treatment of patients' problems and concerns
• To assist in the provision of professional and clinical leadership and expertise in the care of the patient with cancer, acting as a resource to provide information and support to patients, carers and staff.
• To liaise effectively with all members of the multidisciplinary team to achieve a seamless specialist service.
• To ensure all patients have a plan of care and where necessary liaise with members of the multidisciplinary team ensuring that the patients’ treatment pathway is not compromised maintaining the seamless pathway of care.
• To lead and contribute to the creation of patient information
• To provide formal and informal training and education within specialist area
• To lead and contribute to the development of clinical guidelines and policies both internally and externally.
• To attend the Cancer CNS Community of Practice
• Support health and wellbeing events and initiatives to support patients to live with their disease
• Support the wider cancer patient experience projects/ campaign within the Trust

1. CLINICAL RESPONSIBILITIES


• 1 Maintain a professional behaviour at all times and promote a positive image of Royal Free London NHS Foundation Trust at all times, in line with World Class Care values.


• 2 Provide a high quality specialist service for the patient with secondary cancer, from the point of a diagnosis of secondary breast cancer, through treatment and to end of life care disease, ensuring the patient is placed at the centre of care delivery.


• 3 Puts quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, care planning and evaluation of care and all care intervention.


• 4 To discuss all treatment options for the secondary breast cancer with knowledge and expertise to enable patients to make an informed choice.


• 5 Provide appropriate information and psychological support, respecting the diverse cultural backgrounds of patients with courtesy, dignity and privacy.


• 6 Sensitively handle difficult situations that may arise involving patients, carers and health care professionals


• 7 Utilise advanced communication skills to advise and empower other healthcare professionals involved in the care of patients with a diagnosis of Metastatic Breast Cancer




• 8 Attend and actively participate in the main Breast MDT and the Metastatic MDT and work alongside the primary breast cancer team in delivering a seamless service to patients
• 9 Act autonomously and be involved in service development and implement nurse led clinics.


• 10 Develop patient information in line with the Trust patient information policy.


• 11 Attend ward rounds with the oncology team and work closely with the Acute Oncology Service to provide support and advice to patients during any admission.


• 12 Work closely with the Hospital and Community Palliative care Teams – Referring patients appropriately and maintaining communication and promoting collaborative care and support for patients.


• 13 Provide professional, clinical and managerial leadership to nursing and associated staff, and ensure the provision of a high quality, responsive nursing service.


• 14 Collaborate on all aspects of quality management related to the nursing and clinical area.
• 15 Assist in the evaluation of the service and patient care, and monitor patient experience.


• 16 Lead the implementation of improvements in nursing practices within own areas of responsibility in line with local and national agenda and patient feedback.




• 17 To promote new and innovative forms of nursing practice, supported by relevant education and training plans, to deliver care that enhances access to the department and specialist services.


• 18 Maintain awareness of relevant research, evaluations and audit, and act on this information in collaboration with colleagues to continually develop the service and direct and support junior staff.


• 19 Contribute to the creation and design, with other multi-professional team members, care pathways for referral of the particular groups of patients with the aim of disseminating good practice and improving outcomes throughout the trust. Have a thorough understanding of safeguarding procedures and knowledgeable with respect to safeguarding triggers and risks.


• 20 To be professionally and legally responsible and accountable for all aspects of own clinical and professional activities.


• 21 To recognise the limits of own clinical knowledge and skills and to seek medical support whenever needed to provide safe, effective and timely patient care.


• 22 Contribute to the investigation of complaints and untoward incidents and ensure recommendations and related education/training programmes/action plans are implemented.

• 24 Act in accordance with the Nursing and Midwifery Council (NMC) Code of Professional Conduct and Scope for Expanded Practices for Nurses, Midwives and Health Visitors.


2. RESPONSIBILITY FOR PATIENTS

• 1 Act as a keyworker for a clinical caseload, working to coordinate care and provide easy access for ongoing support to patients.


• 2 Provide professional and clinical leadership, providing a highly visible and authoritative presence ensuring the delivery of excellence in core, specialist and advanced nursing practice, audit, research, and service and practice development agendas.




• 3 To identify outcome measures which illustrate the quality of the service with a particular emphasis on establishing ways to show patient related outcome measures.


• 4 Recognises and champions innovations in nursing practice ensuring they are supported, evaluated and contribute to develop an evidence base to meet the needs of the patients, families, relatives and staff.


• 5 Communicates highly complex and sensitive information to patients, carers and other staff.


• 6 Able to diffuse potentially hostile and antagonistic situations with staff, patients and relatives, using highly developed interpersonal and negotiation skills.


• 7 Work with the departmental key stakeholders to rationalise, develop and implement nursing documentation ensuring that practice is standardised and monitored department wide.


• 8 Contributes to the trust’s nursing and professional agenda and promotes the trust as an international centre of excellence and national leader in the speciality.




3. RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT


Work collaboratively with specialist Consultants within the Multidisciplinary team, and take the lead, where appropriate, in identifying innovative ways to improve the service and therefore, patient care. Engage patients and carers wherever possible when developing the service to ensure that their needs and opinions are taken into account and acted upon where appropriate.


• Liaise with other members of the multi-disciplinary team to ensure seamless patient care through the development of protocols and care pathways.


• Contribute to protocols, documentation systems, standards, policies and clinical guidelines which reflect current practices, Trust policies and up to date knowledge, research, and national standards.




• Work with the wider MDT to ensure that service and National targets are met.


• In conjunction with the senior management team participate in business planning cycle, identifying areas of service development, focusing in particular on those related to nursing / education / training / research activities.


• Contribute to organisational development projects to transform clinical services to meet the needs of world class nursing for the future.


4. RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES


• Provide support to the team acting as an expert resource, demonstrating tenacity, drive, professional integrity, balance and perspective.

5. RESPONSIBILITY FOR LEADING AND MANAGING


• Provide professional, clinical and managerial leadership to nursing and associated staff, and ensure the provision of a high quality, responsive nursing service.


• To be fully aware of the targets set out in the NHS plan and Trust and Directorate objectives and in conjunction with the multi professional team use the available resources to meet these targets.


• Contribute to the workforce and financial planning for nursing and support services within own areas of responsibility and where appropriate, work collaboratively across the trust to support these initiatives.


• Engage in the implementation of improvements in nursing practices within own areas of responsibility in line with local and national agenda and patient feedback.


• To have a thorough understanding of safeguarding procedures and knowledgeable with respect to safeguarding triggers and risks.


• To ensure the delivery and compliance of mandatory training requirements for specialist nursing team when appropriate.


6. RESPONSIBILITY FOR INFORMATION RESOURCES


• Contributes to the development of appropriate outcomes and service impact measures to evaluate patient response and service development needs.


• Ensures all information is stored in accordance with the Data Protection Act and Caldecott guidance.


• Identify educational and training requirements on information resources for all relevant staff.


• Process information electronically and manually.


• Ensure information is communicated through the appropriate trust channels as required.


• Oversees high standards of record keeping within areas of responsibility.


7. RESPONSIBILITY FOR RESEARCH, EDUCATION AND DEVELOPMENT

• Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through self-education and attendance at relevant courses of study.


• To undertake relevant research including the measurement and evaluation of the service by the process of audit and patient satisfaction.


• To develop specific audits which support service improvement, or assess the benefits and outcomes following service improvements.


• Provide formal and informal teaching to nurses and AHPs as required.


• Initiate and contribute to the development of resource and teaching packs for use by patients and/ or staff in the clinical and educational settings
