SENIOR CARE WORKER JOB DESCRIPTRION
Job title: Senior Care Assistant
Reporting to: RM
The senior care assistant provides the highest possible standards of care and service to all residents, in addition to performing a supervisory role. Senior care assistants respond to emergencies, providing guidance and support to care assistants under the supervision of Ark Care Principles.
Senior Carer, plays a crucial role in sharing relevant information with professionals to enhance the overall well-being and care coordination for service users. This responsibility must be carried out in strict adherence to data protection and ACP .
This Job role is for 37.5 Hours per week.
Salary: £21060 per Annum

Key Responsibilities:
Collaborative Information Sharing:
• Share pertinent information, where appropriate, with key partners such as healthcare professionals, social workers, and family members.
• Foster open communication channels to facilitate effective collaboration in the best interests of residents.
Data Protection Compliance:
• Adhere to data protection principles and regulations when handling and sharing resident information.
• Ensure that all information shared is done so securely and in accordance with relevant legislation and guidelines.
• Apply Talbert principles when sharing health and social care information, prioritizing the confidentiality and privacy of residents.
• Seek consent from residents and their representatives where required before sharing sensitive information.
Record-Keeping and Documentation:
• Maintain detailed and accurate records of information shared, including the purpose, recipients, and any consents obtained.
• Document the communication process in line with the company's policies and procedures.
Training and Awareness:
• Stay informed about data protection laws, Talbert principles, and other relevant regulations.
• Attend training sessions to enhance awareness and understanding of best practices in information sharing.
• Ensure that all care assistants within your team follow the individualised care plans for residents and encourage them to contribute towards the development and review of these.
• Under the supervision of RM, Care Coordinator, Field Supervisor, support the training and supervision of new and junior staff members in all aspects of their work within the residents’ homes.
• Be aware of CQC fundamental standards.

Management: Keyworker Roles, Supervision and Spot Checks:
• Carry out your key worker duties under the overall management and supervision of your designated line manager.
• Collaborate with the care team to address service users' needs and concern effectively.
• Follow the direction and guidance of the manager in the execution of your key worker duties.
• Seek clarification from the manager when needed and promptly communicate any challenges or concerns.
• Act as the designated key worker for a specific group of nominated service users.
• Foster a supportive and trusting relationship with assigned service users and their representatives.
• Support and mentor junior care staff, providing guidance and sharing expertise.
• Participate in the orientation and training of new team members.
• Conduct regular supervision sessions to assess performance, identify training needs, and ensure adherence to care standards. (Supervision time to be arranged in liaison with the care coordinator for time allocation)
