Summary


Job Title: Community Learning Disability Nurse

Base: Community Learning Disability Service Mile End Hospital London E1 4DG

The Community Learning Disability Service provides integrated specialist health and social care to adults with learning disabilities in Tower Hamlets. Additionally the service is responsible for the provision of a specialist day activity project - CREATE

An exciting opportunity has arisen for two nursing post within the established multi-professional team. You will be expected to:
• To use nursing skills in working with people with learning disabilities and autism to maintain, promote and improve health.
• Be able to assess and support people with learning disability and autism and commonly coexisting conditions like ADHD and mental illnesses.
• Be able to fully involves the person with a learning disability and at risk of behaviours that challenge.
• To support people with learning disabilities to develop their full potential by reducing the impact of their impairment, that could limit their social inclusion.
• To facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.
• To use a person-centred approach to promote choice, independence and inclusion, reflecting the key principles of Valuing People (DOH 2002).
• To contribute to shaping local health services in the way they are organised and run to promote an inclusive service for people with learning disabilities.
• The role involves undertaking reviews of individual’s potential eligibility for NHS Continuing Healthcare Funding in accordance with the National Framework for NHS Continuing Healthcare.
• Understand the implication of the legal and policy framework underpinning the support of people with a learning Disability and autism

Key working relationships:

The post holder is lined managed by the Care Team Manager (Integrated Health & Social Care) – Integrated Multi-professional Team

Post holder is professionally accountable, to the Clinical Nurse Specialist (Lead Nurse)

Maintain multi professional working relationships within CLDS, Primary and secondary care services and other agencies that support people with learning disabilities in Tower Hamlets
