Palliative Care Triage Nurse/Clinical Nurse Specialist, part time - set days

We are pleased to announce an exciting opportunity to join our Specialist Palliative Care Service based at Hemel One, Hemel Hempstead Hertfordshire.

This post holder will triage and coordinate outcomes for all Palliative care referrals that come through our referrals centre for all 3 Palliative Care providers across West Herts.

This role involves triaging and prioritising community, Hospice inpatient and outpatient referrals.

The ideal candidate will be extremely well organised, flexible, approachable, professional and above all compassionate to the needs of patients, carers and professionals within this specialist service and those of other providers.

You will need to have a good sense of humour in order to blend well into the team, a wide range of IT skills, including experience with Systmone, understanding of virtual platform such as Blue jeans and Microsoft Teams, Advanced communication skills and an understanding of the 3 organisations who make up the Palliative Care Providers in this division.

You will need to have good management skills as well as advanced Palliative and end of life care experience.

You will also need experience of working in the community.

• The main component of this job is to be the coordinator of the Palliative Care Referrals Centre (PCRC). This role involves triaging all palliative referrals that come for 3 organisations across West Herts. (CLCH, Rennie Grove Peace (RGP) and The Hospice of St Francis (HOSF).
• Referrals for home assessment, hospice inpatient admission or outpatient assessment all come through this triage process.
• The post holder will be representing all 3 organisations and needs to have a working knowledge of these services.
• This coordination role also includes giving advice to patients/carers and professionals as part of the 24 hour advice line.
