Summary

To provide management support for the KW Primary Care Network to facilitate growth of the PCN and the delivery of services in line with the GP contract and Network Contract Directed Enhanced Service Specification(s) DES. The post holder will contribute to the successful establishment, running and ongoing development of the PCN. In supporting the development of the network, the services required to comply with the necessary contracts specifications etc, investing in relationships between service providers. The post holder will work flexibly across our network practices and closely with the Clinical Director and will support practices to achieve improved outcomes for the local population. The post holder will support the collection of data and information to ensure robust budget monitoring, payment and provision of assurance to the network and/or commissioners relating to any contract compliance. PRIMARY DUTIES AND AREAS OF RESPONSIBILITIES The remit of this role is broad and evolving and will require attention to detail in the delivery of high-quality day to day operations Act as a champion for the network and encourage and support all practices in the network to engage in the process Work with practices within the PCN to identify areas of common concern or potential collaborative working to improve the resilience of general practices and sustain local services Work with and support the Clinical Director to develop and deliver their PCN Plans and performance requirements Continue to support and encourage learning to continually improve network services and patient outcomes. Act as Line Manager for shared PCN staff Company secretary function for the PCN limited company Attend relevant meetings to ascertain a wider knowledge of proposals for change in terms of delivery of health care systems across KW practices and feedback as appropriate. Ensure all relevant national strategies, policies, targets and frameworks (that are agreed by the network steering group) are fully implemented Represent PCN at meetings alongside CD, eg PCS meetings with citywide networks etc. Management Encourage practices to create and develop ideas and provide suggestions as to how they can deliver the GP contract and DES specification to ensure the success of the network using the strengths and relationships with internal and external stakeholders. Identify any risks and barriers with practices and work to mitigate or overcome these where possible. Support practices with formulating project and/or delivery plans in relation to meet the GP contract/DES specification, including writing plans and business cases on behalf of the network as appropriate. Support the review of PCN agreements as and when required including reflecting changes to practice members and the GP contract/DES specification(s) as these are issued by NHS England. Understand risks and identify these for the Clinical Director, and help to identify any mitigating actions to reduce risk. Undertake a lead role, as required, to ensure the Network complies with GDPR Ensure that patient safety is at the forefront of all service developments and delivery. Support the recruitment, induction, training and appraisal of new and existing staff groups/members employed on behalf of the network. Leadership Support the Clinical director in developing and setting shared purpose and vision Develop Engagement in line with shared purpose, and local and national stated priorities Ensure the functioning of the network align to the agreed shared purpose of the PCN Pursue the dual functions of PCNs to stabilise general practice, and develop holistic, integrated (health and social) care. Resource Management Manage resources in order to formulate, mobilise and ensure the ongoing implementation of plans to any agreed specification Organise meetings including steering group and others as they arise, minute taking and progressing any action points Organise PCN events Identify potential sources of funding to support the network and its plans and submit bids as appropriate. Ensure that specifications, service contracts and/or any plans are adequately monitored and on schedule and that this information is appropriately communicated to relevant stakeholders. Oversee any sub-contracting arrangements, negotiating effective use of resources, agreeing service level agreements or contracts and monitoring delivery on behalf of the respective networks. Attend meetings as appropriate to the role. These may be outside of normal hours. Reporting Provide information and updates to the PCN Clinical Director of the developing PCN agenda and what is happening within and across the other networks in Sheffield. Provide routine reporting, including budget monitoring, to the respective PCN steering group (in accordance with their respective agreements). Relationships The post holder will have constructive relationships with the following internal and external stakeholders: Clinical Director for the Network. Practices within the PCN and across Sheffield. PCN leads at the ICB. Other network managers in Sheffield Other agencies eg Age UK Financial Management: Manage all delegated budgets with propriety in line with all relevant organisational procedures with a view to achieving the best impact from well managed finances In conjunction with the Clinical Director review, authorise and approve operational services spend, ensuring a fair and reasonable allocation of funds Ensure financial risks are recognised and appropriate action taken Relationship Management: Manage and maintain credibility with all key contacts within the network, to foster a culture of collaboration, and promote inter-service learning to drive continuous improvement Build good relationships with all stakeholders involved in the PCN Develop and deliver a communication strategy with reference to the networks geographical spread Represent the network at meetings, in the community and to the media as required Listen and act on staff feedback from the frontline to build staff trust by adopting a proactive, facilitative approach. Ensure their representation and commitment across the PCN. Provide coaching and support to staff and stakeholders involved in PCN transformation and development CONFIDENTIALITY The post holder may have access to confidential information relating to network practices, patients, their carers, practice staff and other healthcare workers. They may also have access to information relating to the Network as a business organisation. All such information from any source is to be regarded as strictly confidential.
