Summary


Ward B7 is a 32 bedded Hip fracture frailty ward under dual responsibility of Orthogeriatricians and Hip surgeons.

To support the clinical staff in the smooth running of the ward, book transport, liaise with capacity coordinator, Pharmacy.

This role will also include general ward clerk duties.
• To support the ward manager and ward co-ordinator/NIC in maintaining patient flow.
• To support the clinical staff in keeping e-track up to date.
• Liaise with the duty manager when a bed is available.
• Attend Whiteboard rounds, identify actions required and actively manage agreed discharge plans for patients on the ward.
• Maintain whiteboards and e-track information to ensure it is up to date.
• Arrange or where necessary photocopy notes for patient transfer.
• Assist in the completion of discharge check lists to ensure accurate discharge planning is evident.
• Chase doctors for completion of discharge letter and deliver TTOs if necessary.
• Ensure diagnostic requests are expedited promptly and escalate any delays to the appropriate person. .
• Ensure appropriate transport is arranged for discharge.
• Ensure patients have had the opportunity to complete a friends and family feedback survey.
• Utilise IT systems to secure and record timely and accurate data relevant to the role and function.
• Coordinate admissions and transfers in the wards, and external transfers to other organisations.
• Complete the filing of patient notes when discharged.
