Summary

Principal Responsibilities

1 Build and sustain relationships with young people who are preparing to leave care to ensure their views are taking into account and that practice is centred on meeting their needs.

2 Participate in assessment, preparation and review of care leavers' pathway plans to equip them with the emotional, social and financial skills required for successful independent living.

3 Co-ordinate the provision of services and take reasonable steps so that care leavers make use of these services.

4 Maintain effective communication with partner agencies and other professionals in the development of services and to keep informed about care leavers' progress and wellbeing.

5 Keep full, accurate and up-to-date records of contact with the care leaver and services provided using the Council's electronic recording system.

6 Provide a rapid and flexible response to supporting in crisis; which may include out of normal office hours and occasional weekend working.

7 Promote the Council's Equal Opportunities Policy in the delivery of services and treatment of others by maintaining a caring ethos that takes account of young people's race, culture, disability, gender and sexuality.

8 Work towards local and national good practice guidance and respond to local and national strategic and operational agendas in the development and delivery of the service. IMPORTANT THE REHABILITATION OF OFFENDERS ACT The provisions of the Rehabilitation of Offenders Act relating to the non-disclosure of spent convictions do not apply to this job, YOU MUST, THEREFORE, DISCLOSE WHETHER YOU HAVE ANY PREVIOUS CONVICTIONS WHERE PROMPTED TO DO SO WHEN APPLYING FOR THIS JOB. If successful, you will also be required to apply for a Criminal Record Check from the Disclosure and Barring Service. The level of check required for this job is Enhanced Disclosure.

9 Promote the best interests of children and young people by acting as an advocate for their individual rights, and safeguard from harm by invoking, where necessary, CEC's Child Protection, safeguarding and complaints procedures, taking any other measures to enable the well being of children and young people.

10 Participate in all aspects of performance management, supervision, PDR reviews, training and development and use all relevant learning opportunities to improve skills and knowledge.

11 Comply with the Council's Health and Safety Policy and associated safe working procedures and guidelines. The job holder may be required at any time to undertake any work up to and/or at a level consistent with existing responsibilities. These tasks may be at any location in Cheshire East, to ensure the effective deployment of labour, materials, transport and equipment to meet daily service requirements.
