Summary
Undertake clinical medication reviews with patients Carry out Structured Medication Reviews with patients Reconcile medications following patients discharge from hospital/clinic appointments Implement and embed a robust repeat prescribing system Design, develop and implement practice searches Ensure patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time Encourage cost-effective prescribing within the practice Analyse, interpret and present medicines data to highlight issues and risks to support decision making Identify and provide leadership on areas of prescribing and medicines optimisation
