Summary

To develop and provide a high quality Clinical Pharmacy Service to ECCH Primary Care Home Teams (PCH) including individual patient medicines review, counselling, medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimised. To support the provision of clinical pharmacy services to the community hospital ward and specialist palliative care beds. Participate in ward rounds and MDTs where appropriate. To provide professional expertise in relation to drugs or medicines to PCH staff; patients and carers and others including medical practitioners. To support and implement efficient medicines management initiatives to ensure the safe and secure handling of medicines; reduce drug waste and manage drug expenditure. To prescribe within sphere of competence and where appropriate the necessary medicines to meet the needs of the patient. To participate in MDTs and PCH huddles where appropriate. To work closely with PCH pharmacy technicians to enable patients in our care to be empowered to manage their medicines. To facilitate the medicines element of discharge to enable safe and efficient discharges. To participate in and undertake audits as agreed including collecting and recording data for KPIs. To support the wider pharmacy and ECCH teams in delivering safe patient centred care through implementation of the Pharmacy and Medicines Optimisation strategy including providing professional advice; training and education; developing and reviewing relevant policies and SOPs. To manage PCH based pharmacy technicians.
