At Thames Reach we run projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We support vulnerable people to prevent them from becoming homeless. We help formerly homeless people get their lives back on track. We help people gain independence, through finding work, accessing training and engaging in volunteering opportunities.

About this role…

Thames Reach’s hostels are transitional spaces where a 24 hour staff team support people to move in, assess their needs, and quickly identify the best place for them to move on to next. Skilled staff teams develop a personalised support plan to ensure people are ready for their move on and have holistic support with their mental and physical health, substance misuse needs, criminal justice issues and life skills.

We are looking for Support Workers to join two of our hostel teams, Acre Lane and Robertson Street.

As a Support Worker, you will:

You will:

- Conduct Risk Assessments, Needs
- Assessments and Support Plans with clients in the service.
- Liaise with external agencies and councils to ensure clients are receiving appropriate support.
- Support and encourage clients to become more independent.
- Support our clients’ aims and help them to move on from the hostel within set timeframes.
- Ensure the environment is safe, healthy, reflective of our ethos and supports our client’s aims.
- Be outcome focussed, fostering a culture of high performance and continual improvement on your and the team’s successes.

To succeed as a Support Worker, you will be:

- Solution and outcome focussed with a commitment to supporting people to achieve the best outcomes as they work towards independence
- Organised and efficient in your work.
- Excellent at communicating and problem solving.
- Confident in your ability to support clients.
- Competent at using a computer and a range of software packages
- Committed to developing your knowledge and skills
