Summary
Provide specialist pharmaceutical information to medical and nursing staffand patients on the designated ward(s) and to General Practitioners andother staff within the Trust where appropriate. Participate in consultant ward rounds and multidisciplinary meetings relevant to the designated ward(s). To participate in the therapeutic drug monitoring service provided inaccordance with departmental procedure. Undertake clinical audit and practice research within the PharmacyDepartment. Record regularly clinical interventions and activities in line with departmental policy. Provide professional and legal clinical supervision and act as the designatedResponsible Pharmacist in the dispensary as allocated. Prepare the weekly pharmacist dispensary cover rota in line withdepartmental policy. Investigate any medicines related incidents or complaints from the designated clinical area and explore ways of minimising risk of future re-occurrence orrisk to staff. Liaise with appropriate health care professionals within Primary Care Truststo ensure the effective and efficient use of resources across theprimary/secondary care interface and where necessary develop shared careprotocols.
