Summary


The role of the Quality Improvement Lead for Care Homes is to lead and support Quality Improvement (QI) projects across Care Homes within Calderdale. They will play a vital role in identifying projects and priorities requiring a focused QI approach in individual service delivery settings and across the system.

The post holder will work with partners using established mechanisms and relationships to gather both data and intelligence to allow identification, prioritisation and delivery of QI projects which achieve measurable sustained outcomes.
• Work with system partners to identify opportunities for QI projects.
• Lead and support QI programmes across care homes and domiciliary care within Calderdale.
• Provide accurate evidence-based education and specialist support to care staff including quality improvement often in complex care settings.
• Work with care home and domiciliary care managers and staff, adopting a proactive approach to collaboration, using quality improvement tools to facilitate change.
• Lead on the development of regular training/updates/feedback to staff
• Develop and contribute towards implementation of project plans to achieve outcomes within identified timescales.
• Plan, design, implement surveys or audits to assist in the development and improvement of care delivery including evaluation of the QI interventions as each individual work stream requires or is detailed within the project plans.
• To support colleagues to ensure effective and efficient delivery of the QI programmes
• To provide specialist advice and support the delivery of high quality holistic, evidence-based education via the QI programmes to Care Homes and Domiciliary Care agencies that is responsive, effective and efficient
• To support the implementation of QI across the Calderdale care home and domiciliary care sector
• To liaise effectively with all other health care professionals involved in the residents care where required

• To take responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements.
• Demonstrate knowledge of safeguarding procedures and policies for adults including MCA and DOLS.
• Inspire an interest in QI approaches, having a communication style which engages, motivates and enables people.
• Develop and maintain good relationships, communication and teamwork with all stakeholders.
