About the role
An exciting opportunity has arisen in Pond Tail Surgery for a Nurse Practitioner to join the team permanently, working for Dorking Healthcare. The main aim for this job role is to share workload and responsibility with General Practitioners, providing a service complementary to GP’s and other ANPs making best use of clinical skills within the practice, as well as being expected to work with the Primary Health Care team to ensure a seamless delivery of care within the practice and to promote a positive image of the practice to its patients and service users.

Duties include: providing a holistic, autonomous and clinical nursing service by attending to referrals and self referrals of practice patients. You will be assessing patients’ physical, psychological, and social needs, implementing agreed management plans with patients including appropriate prescribing and referral, diagnosing, discharging, or referring independently. Setting standards of care by implementing protocols and encouraging new health promotion activities within the practice. You will also autonomously manage chronic disease with the support of the GP’s and also be an Independent Nurse Prescriber if you are qualified. You will also carry out triage of patients and will be required to maintain and collage statistical information for both ad hoc and regular reports for audit purposes and to co-operate and participate in research projects agreed by the Primary Health Care Team.

DHC GP Federation is a patient focused and pro-active community-based provider with strong links to all the GP’s working within our Federation of 3 Primary Care Networks including 9 GP Practices (soon to be 10 with the addition of Pond Tail Surgery) across Surrey from Dorking to South Tandridge.

We have recently been awarded the contract to run Godstone GP Practice from 1st January 2022 and are looking to increase the team of clinicians and GP’s working within the Practice to re-establish the service after a period of considerable change. About you

You need a “can do” and enthusiastic approach to work within DHC and a real commitment to delivering first class services to our patients.

It is essential that the post holder has proven recent experience working as a Nurse Practitioner, able to triage by telephone and be organised with the ability to deal with a busy workload. The successful candidate must be able to prioritise and organise their own workload, have good IT skills, exercise initiative and independent judgement whilst maintaining a high level of diplomacy and confidentiality at all times.

  • Have a relevant postgraduate qualification
  • Excellent organisational skills
  • Clear speaking voice
  • Have a positive approach and ability to work under pressure

  • Ability to cope calmly with a busy workload
  • Have good IT skills with the ability to use patient data systems and other Microsoft packages, Word, Excel and E-mail.
  • Ability to form and maintain good working relationships with colleagues and patients.
