We are seeking a motivated and dynamic Band 4, Qualified Nursing Associate to work within our Primary Home Care Team - North. You will be based at Twinwoods Health Resource Centre (Clapham), however, there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to be one of the first Nursing Associates to join our Community Nursing Teams. To develop your knowledge and skills within the community nursing setting, with the possibility of being supported to complete your Nursing qualification. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion, and innovative activities and projects that enhance patient service delivery and staff wellbeing.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important.
You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.
