Summary


This is a full time Band 4 Frailty Coordinator post working in a newly established frailty service at the Oldham Care Organisation. This role is part of a key development in delivering specialised frailty input to frail, older patients presenting to ED.

The Coordinator will provide administrative and operational support for the Frailty team based at the Royal Oldham Hospital. They will be the first point of contact for the frailty service, assisting in the smooth and efficient running of the frailty service and will be included in the 7 day working roster, which involves working up to 1 weekend every 4 weeks. The successful candidate will work within a team that consists of a Geriatrician, Advanced Clinical Practitioner, Therapists, Nurse, Social Worker and Pharmacist.
