Summary


Band 5 Community Occupational Therapist

We are looking to recruit a Band 5 Occupational Therapist with a patient-centred approach and a passion for rehabilitation to our Erewash Integrated Community team.

You will be working within an established Community Therapy Team, which is closely aligned to Community Nursing, the Frailty Service and the Erewash Care Team.

You will have the opportunity to develop your holistic assessment skills and be exposed to patients who present with a wide variety of conditions.

You will be required to work alongside our Rapid Service, for which you will be part of a 7 day working rota. This will enable you to develop your ability to responsively assess and provide intervention, meeting new national waiting time expectations.

There is a strong focus on rehabilitation with the need to prevent hospital admission and facilitate discharge.

As part of the role there will be a need for travel and transportation of equipment across the whole of Erewash.

You will have the opportunity to develop your holistic assessment skills and be exposed to patients who present with a wide variety of conditions.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
