Summary


Wolverhampton Community Intermediate Care Service is looking for a Rehabilitation Assistant to join our multidisciplinary team based at West Park rehabilitation hospital

Our team includes Nurses, Occupational Therapists, Physiotherapists, Rehabilitation Assistants, Senior support workers and Therapy Technicians. The Community Intermediate Care Team (CICT) provides short term (up to six weeks) rehabilitation and re-enablement programmes to patients in their own homes.

You must have:
• NVQ 3 in health and social care or equivalent
• Experience of working in intermediate care or rehabilitation setting
• Experience of providing rehabilitation support programmes
• Have a full UK driving licence and vehicle that can be used for work
• Community experience is desirable

A flexible approach is required with the post holder possessing effective communication and problem solving skills.

To support members of the multi-disciplinary team in providing a high quality of care and rehabilitation to patients in the community. To work on assigned tasks, often unsupervised, delivering care as a lone worker.

To contribute positively to the continuing development of the service and the team.

To contribute to maintaining the independence of patients in their own environment.
